PDCA循环在降低静脉用药调配中心差错中的应用

Application of PDCA cycle in reducing errors in pharmacy intravenous admixture service

目的:在静脉用药调配中心(pharmacy intravenous admixture service,PIVAS)实行PDCA管理,提高员工参与管理的意识,减少各类差错的发生。方法:运用PDCA循环,对各类差错产生的原因进行分析,设定目标,制定相应的防范措施,逐一改进。结果:通过2年时间的持续改进,PIVAS的外部差错率由0.02%下降至0.000 8%,内部差错率由0.18%下降至0.09%,均显著降低。结论:PDCA循环能显著降低PIVAS的配方差错率,减少潜在的医疗风险,保障患者用药安全。

Objective： To enhance the employees＇ management awareness and reduce incidence of errors in pharmacy intravenous admixture service（PlVAS） by plan do-check action（PDCA） cycle. Methods： Reasons for various errors were analyzed, targets were set, corresponding preventive measures were taken, and errors were corrected one by one in the PDCA cycle. Results： Through continuous improvement for two years,external error rate was decreased from 0.02~ to 0. 000 8~ and internal error rate was decreased from 0. 18% to 0.09%. Conclusion： PI）CA cycle could significantly decrease the incidence of errors in PIVAS, reduce potential medical risks,and ensure the safety of drug use in patients.
