HKV、DR及HRCT在尘肺病诊断中应用研究

Study on the Application of HKV, DR and HRCT in the Diagnosis of Pneumoconiosis

目的探讨高千伏胸片(HKV)、数字X线成像系统(DR)及高分辨率CT(HRCT)在尘肺病诊断中的应用价值。方法选择本院受理的184例疑似尘肺病患者,同时实施HKV、DR及HRCT摄片,将其结果进行比较。结果HKV与DR在尘肺病诊断上差异无统计学意义,但DR胸片一级优片率(91.30%)明显高于HKV(63.04%)(P<0.05)。HRCT在尘肺Ⅰ期、尘肺Ⅲ期的诊断率及尘肺并发症的判断方面均高于HKV(P<0.05)。结论 DR在尘肺病中诊断技术质量优于传统的HKV,HRCT在尘肺病的早期发现、小阴影聚集及尘肺并发症的诊断中具有重要意义。

Objective To explore the values of high kilovoltage chest radiograph （HKV）,digital X-ray imaging system （DR） and high-resolution CT （HRCT） in the diagnosis of pneumoconiosis.Methods 184 patients suspected with pneumoconiosis in Hunan Prevention and Treatment Institute for Occupational Diseases were examined by HKV,DR and HRCT techniques,and the results were compared.Results HKV and DR were not significantly different in the diagnosis of pneumoconiosis.However,the rate of grade-Ⅰ film of DR chest film （91.30 ％） was statistically higher than that of HKV（63.04 ％,P ＜0.05）.HRCT was superior to HKV in the diagnosis rate and determination of pneumoconiosis complications in stage 1 and 3 of pneumoconiosis.Conclusions DR is better than the conventional HKV in technical quality in dignosing pneumoconiosis.HRCT is of great importance in the early detection of pneumoconiosis,diminutive shade aggregation and in the diagnosis of pneumoconiosis complications.
