笑氧混合气体吸入镇痛在结肠镜诊疗中的应用效果 被引量:2

摘要 目的研究笑氧混合气体吸入镇痛在结肠镜诊疗中的应用效果与安全性。方法将928例须行结肠镜检查或治疗的患者按随机数字表法分为A组466例(其中行结肠镜检查436例、行结肠镜下息肉切除术30例)、B组462例(其中行结肠镜检查434例、行结肠镜下息肉切除术28例)。A组行结肠镜检查或治疗时采用笑氧混合气体吸入镇痛;对照组按常规操作。观察2组操作期间患者心率(HR)、血氧饱和度(SpO2)、血压(BP)的变化以及镇痛效果、操作时间、不良反应发生的情况。结果 A组操作中、操作后患者HR、BP较B组明显降低(均P<0.05);在操作过程中A组患者出现腹胀、恶心症状的例数及操作时间明显少于B组(均P<0.05);A组镇痛效果明显优于B组,且患者愿意再次接受检查的比例明显高于B组(均P<0.05)。结论笑氧混合气体吸入镇痛在结肠镜的诊疗应用中具有操作简便、安全、起效快的特点,总体效果良好。 Objective To investigate the efficacy and safety of inhalation analgesia with nitrous oxide and oxygen mixture in coldnoscopic diagnosis and therapy. Methods A total of 928 patients undergoing colonoscopy or treatment were randomly divided into A group (436 patients undergoing colonoscopy and 30 patients undergoing colonoscopic polypectomy)and B group (434 patients undergoing colonoscopy and 28 patients undergoing colonoscopic polypectomy). Inhalation analgesia with nitrous oxide and oxygen mixture was performed in A group and conventional anesthesia in B group. Heart rate (HR), oxygen saturation (SpO2), blood pressure (BP), analgesic effect, operation time and adverse reactions were observed in both groups. Results Compared with B group, the HR and BP reduced during and after operation and the incidence of abdominal distention or nausea and operation time decreased during operation in A group (P〈O.05). In addition, the analgesic effect was improved and the proportion of patients willing to receive examination again increased in A group, compared with B group (P〈0.05). Conclusion Inhalation analgesia with nitrous oxide and oxygen mixture is simple, safe and effective in colonoscopic diagnosis and therapy.
