腰背部原发性皮肤弥漫大B细胞淋巴瘤

Primary cutaneous diffuse large B-cell lymphoma of the back

报告1例发生于腰背部的原发性皮肤弥漫大B细胞淋巴瘤。患者女,46岁。腰背部肿块1年余。皮损组织病理学检查示:表皮厚度大致正常,真皮乳头水肿,可见一无浸润带,真皮全层灶性或弥漫性单一核细胞浸润,细胞异形,部分细胞核细胞质比例大,核仁清晰,并可见多数核分裂象。免疫组化示:CD20、CD45RO、CD79a、多发性骨髓瘤原癌基因(MUM)-1、配对盒基因(PAX)-5阳性。诊断:原发性皮肤弥漫大B细胞淋巴瘤。治疗:予以R-CHOP(利妥昔单抗、环磷酰胺、吡柔比星、长春新碱、泼尼松)方案化疗,肿块较前缩小。

A case of primary cutaneous diffuse large B-cell lymphoma is reptmed. A 46-year-old female had a mass on the back for more than 1 year. Skin biopsy showed normal epidermis, papillary, dermal edema, grenz zone, focal or diffuse mononuclear cell infiltration, atypical and large nuclei cells with clear nucleus and mitosis, lmmunohistochemical staining showed positive for CD20, CD45Ro, CD79a, MUM-1 and PAX-5. The final diagnosis was primary cutaneous diffuse large B- cell lymphoma. After the treatment with systemic chemotherapy （R-CHOP）, tumor hecame smaller.
