摘要
目的 :探讨在颈椎间盘纤维环及髓核退变中生化成分的变化。方法 :通过正常人、单纯颈椎间盘突出症、脊髓型颈椎病 3组椎间盘纤维环及髓核生化成分测量 ,主要测定水分、胶原蛋白及蛋白多糖的含量。结果 :单纯颈椎间盘突出症与正常人椎间盘内生化成分无明显变化 (p >0 .0 5 ) ,而在脊髓型颈椎病人的退变间盘中的生化成分较正常人有明显变化 ( p <0 .0 5 ) ,主要表现为前者退变间盘中水分及蛋白多糖含量明显减少 ,而胶原蛋白明显增加 ,但Ⅰ /Ⅱ型胶原比无明显变化 ( p >0 .0 5 )。结论 :单纯颈椎间盘突出为退变的较早期阶段为可逆期 ,脊髓型颈椎病为退变晚期阶段 ,为不可逆期 ;颈腰间盘退变不完全相同。
Objective: To study the biochemical component changes of the normal and degenerated cervical intervertebral discs.Methods:Specimens were taken from 4 normal adults,2 patients with acute cervical intervertebral disc herniation (CDH) and 8 patients with cervical spondylosis myelopathy(CSM).Anulus fibrosus (AF) and nucleus pulposus(NP) were isolated to measure the water,collagen,glycosaminoglycans content and the ratio of type Ⅰ to type Ⅱ collagen.Results:There were no significant biochemical changes in CDH patients.In the AF of CSM patients,the contents of collagen,glycosaminoglycans and water changed dramatically.The contents of collagen,hexosamine and galactosamine in NP of CSM patients decreased significantly.There was no changes in the ratio of type Ⅰto type Ⅱ collagen between the patients and control.Conclusion:There were no remarkable biochemical changes in CDH that is the reversible phase of degeneration.Significant biochemical changes were found in CSM tha tis the irreversible phase.The degeneration of cervical intervertebral disc is different from that of the lumbar intervertebral disc.
