新型农村合作医疗对农民卫生服务利用的影响研究

The impact study on farmers' health service utilization under new co-operative medical system

[目的]分析新型农村合作医疗制度的实施对于提高农民卫生服务利用状况的作用和影响程度。[方法]利用中国健康与养老追踪调查数据,采用回归分析法分析个人因素、经济因素、新农合制度因素和外界因素对农民卫生服务利用的影响。[结果]健康状况和教育程度显著影响了农民卫生服务利用率;新农合制度的实施增加了住院医疗服务的利用率,但在提高门诊服务利用率上作用有限。[结论]新农合制度建设和运行效果取得了明显成效,参合农民的卫生服务利用率明显提高,但仍面临报销手续繁琐、门诊服务利用率不高等问题。建议扩大门诊覆盖范围,提高门诊报销比例,提高参合者的教育程度,增加人力资本等措施提高新农合的卫生服务利用率。

Objective Analyzes the function and impact of improving farmers＇ health service utilization status under new co-operative medical system（NRCMCS）. Methods Takes advantage of Chinese health and retirement health service investigation data, analyzes the individual, economic, NRCMCS and external factors on farmers＇ health service utilization. Results The health status and degree of education have significant impact on farmers＇ health services utilization rate. The implement of NRCMCS increases the utilization rate of hospitalized medical services, but it has limited to increases utilization rate of outpatient services. Conclusions The NRCMCS construction and operating effect have significant results. The utilization rate of health services of the farmers who takes part in NRCMCS has obviously increased. But they still face the problem of cumbersome reimbursement procedures, lower utilization rate of outpatient services. Therefore, we suggest that enlarge outpatient coverage area, increases the proportion of outpatient reimbursement, increases education level of farmers who takes part in NRCMCS and human capital, so as to improve health services utilization rate under NRCMCS.
