中分子羟乙基淀粉对高海拔中重度卵巢过度刺激综合征预防作用研究

摘要 目的探讨中分子羟乙基淀粉(6%,130/0.4)对高海拔卵巢过度刺激综合征(OHSS)中重度OHSS高危患者发病的预防作用。方法将2012年1月~2012年11月在高海拔地区行体外受精-胚胎移植(IVF-ET)治疗85个周期的OHSS高危患者(HCG日血清雌二醇≥4000 pg/mL或获卵数≥15个)随机分成两组:A组为研究组自取卵后给予中分子羟乙基淀粉;B组为对照组自取卵后行常规抗菌治疗。对两组临床及实验室资料进行统计学分析。结果研究组共42个周期,妊娠率为39.47%,周期取消率9.52%,无中重度OHSS的发生;对照组共43个周期,妊娠率为37.84%,周期取消率13.95%,中重度OHSS的发病率为4.65%;两组间比较各项指标及妊娠率、周期取消率、中重度OHSS的发病率间均无统计学差异(P>0.05),但对照组的中重度OHSS发病率及周期取消率均高于研究组。结论中分子羟乙基淀粉对高海拔中重度卵巢过度刺激综合征具有一定预防作用。 Objective To investigate the preventive effects of hydroxyethyl starch molecules on high altitude moderate or severe ovarian hyperstimulation syndrome (OHSS). Methods Retrospective analysis was done from Jan- uary 2012 to November 2012 in our center using in vitro fertilization and embryo transfer( IVF - ET)in high alti- tude. The high -risk patients with severe ovarian hyperstimulation syndrome (HCG day serum estradiol was 4000 pg/mL or the number of eggs were 15 )were treated by 85 cycles and divided into two groups:Group A for the team the eggs were taken by ourselves and treated by hydroxyethyl starch molecules from July 2012 to November 2012; Group B as control group, conventional antibiotic treatment was received after eggs taken. The data from clinical or Lab were done statistics analysis. Results 42 cycles were observed in group A. The pregnancy rate was 39.47%, and the periodicity cancel was 9.52%, no moderate or severe OHSS was occurred. 43 cycles were observed in group B.The pregnancy rate was 37.84%, and the periodicities cancel was 13.95%. The incidence of moderate or severe OHSS was 6.98% ,No severe OHSS was occurred. There were no statistical difference( P 〉 0. 5 )between the two groups concerning various indexes, pregnancy rate, the periodicity cancel rate and the incidence of moderate or se- vere OHSS. But the incidence of moderate or severe OHSS and cycle cancellation rate were higher in group B than the group A. Conclusion There are some preventive effects of hydroxyethyl starch molecules shown on high altitude moderate or severe ovarian hyperstimulation syndrome (OHSS).
