摘要 目的:探讨食物不耐受特异性抗体检测在婴儿湿疹疾病诊疗中的作用,为诊断、预防和治疗婴儿湿疹提供更好的手段。方法:采用ELISA法检测228例婴儿湿疹患儿血清中的特异性IgG抗体,并根据病史、临床症状及血清特异性IgG检测结果剔除相应食物,观察随访患儿临床症状改善情况。结果:14种食物中对鸡蛋的特异性比例最高209例(91.7%),其次为牛奶159例(69.7%),小麦88例(38.6%),大豆61例(26.8%),鳕鱼38例(16.7%),大米31例(13.6%)蟹25例(11.0%),虾3例(1.3%),蘑菇和西红柿各2例,分别占0.9%,牛肉、猪肉、玉米分别为1例,分别占0.4%。对特异性IgG阳性程度达++及以上患儿采取忌食过敏食物治疗3~6个月,并予回访。症状明显改善182例(79.82%),症状改善40例(17.54%),无效6例(2.63%)。结论:检测血清食物特异性IgG,进而了解患儿食物不耐受情况,可为婴儿湿疹诊断、预防及调整饮食治疗提供更客观的依据。 Objective: To explore application of food intolerance specific antibody on diagnosis and treatment of infantile eczema. Methods: The specific antibodies of serum of 228 in/ants with eczema were detected with ELISA. The suspected allergy foods were rejected during feeding and the clinical symptoms were observed. Results: There were 209 cases (91.7%) with egg specific antibody, which was the highest among the 14 foods. The rate of milk, wheat, soybean, cod, rice, crab, shrimp, mushrooms, tomato, beef, pork and corn specific antibody were 69. 7%, 38. 6%, 26.8%, 16. 7%, 13.6%, 11.0%, 1.3%, 0. 9%, 0. 9%, 0. 4%, 0. 4% and 0. 4%, respectively. After 3-6 months treatment of prohibiting allergy food, there were 182 cases (79.82%) with obvious improvement of clinical symptoms. Conclusion: Measurement of food specific IgG is necessary for diagnosis, prevention and treatment of infantile eczema.
