摘要 目的 探讨多普勒超声肾血流测定对诊断高血压病早期肾损害的价值。方法 以血、尿 β2 微球蛋白 (β2 MG)和尿白蛋白排泄率 (UAER)作为早期肾损害指标 ,对 6 0例高血压病患者和 2 0例正常人行多普勒超声肾血流检查。结果 叶间动脉舒张期末血流速度 (Vd)的减慢、血流峰速加速时间的延长是高血压病患者最早出现的肾血流动力学异常 ;有肾脏早期损害的高血压病患者肾血流频谱参数特点是肾段动脉、叶间动脉的收缩期最大血流速度 (Vs)、Vd 减低 ,搏动指数 (PI)、阻力指数 (RI)明显增高。结论 多普勒超声肾血流测定是诊断和预测高血压病早期肾损害的简便易行。 Objective To evaluate the value of ultrasound Doppler renal blood flow measurement in diagnosing early renal damage of hypertension.Methods Sixty patients with hypertension and twenty normotensive subjects were included in the study. They were classified into three groups according to β 2 MG contents in blood or urine and urinary albumin excretion rate (UAER).Renal blood flow was measured all in patients by Doppler ultrasound. Results The earliest abnormal of renal blood flow occured in hypertensives included that the end diastolic velocity (V d) of interlobar artery was decreased and the acceleration time (AT) of segmental and interlobar artery was increased.Peak velocity in systole(V s) and V d decreased and pulsed index (PI) and resistance index (RI) inceased in segmental and interlobar artery of all hypertensives with early renal damage.Conclusions Doppler Ultrasound renal blood flow measurement may be clinically useful in the noninvasive evaluation of early renal damage of hypertension.
