摘要
目的探讨CT灌注成像技术在脊柱病变定量、定性诊断中的作用。方法对125例脊柱病变患者进行CT灌注成像扫描,计算并分析感兴趣区的灌注参数值,包括血流量(BF),血容量(BV),平均通过时间(MTT)和表面通透性(PS)。对照实际病理结果,分为良性病变和恶性病变两组,定量、定性分析良、恶性肿瘤的灌注成像规律。结果良、恶性病变组病变区域的BF、BV和PS值为均明显高于各自正常组织,而MTT值小于正常组织(P<0.05)。恶性病变组病变区的BF值和PS值明显高于良性病变组(P<0.05),而BV值和MTT值无统计学差异。结论 CT灌注成像是一种在定量、定性分析脊柱病变血流灌注状态方面具有重要价值的功能成像方法。
Objective To investigate the clinical value of MSCT perfusion imaging in diagnosing spinal affections. Methods 125 patients suspected spinal affections .were underwent MSCT perfusion imaging. Perfusion parameters, in- cluding the blood flow (BF), blood volume (BV), mean transit time (MTT), permeability surface (PS), were compu- ted and analysed. Compare with the pathological findings, patients were divided into the benign and malignant affection groups. Comparisons of perfusion parameters among the groups and within group between affection and surrounding bones or tissues were carried out both quantitatively and qualitatively. Results For affection between patient groups, the BF and PS values had statistical significant difference, while BV and MTT values showed no statistical difference between benign and malignant affection. For individual patient within group with either benign or malignant affection, their BF, BV and PS values were significantly higher than those for surrounding bones or tissues, while MTT values were significantly lower, with P d0.05 in both comparisons. Conclusion MSCT perfusion imaging demonstrated clini- cal value in diagnosing spinal affections. From perfusion analysis we may separate benign and malignant affections both qualitatively and quantitatively.
