女性膀胱颈原发性恶性黑色素瘤1例报告并文献复习

Primary malignant melanoma of female bladder neck: a case report and literature review

目的探讨女性膀胱颈原发性恶性黑色素瘤的诊断及治疗。方法回顾性分析我科收治的1例女性膀胱颈原发性恶性黑色素瘤的临床资料。结果患者43岁,以排尿困难伴血尿2个月就诊,行膀胱颈肿块电切术,术后病理诊断为膀胱颈口恶性黑色素瘤,免疫组化提示HMB45、S-100、Melan-A及P53阳性。结论女性膀胱颈原发性恶性黑色素瘤极为罕见,确诊需依靠病理学检查。治疗主张行膀胱根治性切除和盆腔淋巴结清扫术,辅以放化疗及免疫治疗。本病预后极差,应早诊断,综合治疗。

Objective To study the diagnosis and treatment of the primary malignant melanoma of female bladder neck. Methods A retrospective analysis was performed in one case of primary malignant melanoma of female bladder neck. Results The patient was 43 years old, complained of dysuria and hematuria for two months. The case was undertaken transurethral resection of bladder neck mass. The patho-logical diagnosis was malignant melanoma of bladder neck, and the expression of HMB -45, S - 100, Melan -A, P53 were seen in the tumor cells. Conclusion Primary malignant melanoma of female bladder neck is a rare disease, and the diagnosis should depend on the pathological examination. The treatment advocates radical cystectomy of bladder and pelvic lymph nodes dissection, supplemented by radiotherapy, chemotherapy and immunotherapy. The prognosis of the disease is very poor, thus it should be diagnosed early and comprehensively treated.
