摘要
目的探讨中频离子导入癌痛消结合三阶梯止痛法治疗癌痛的临床疗效和安全性。方法将80例中重度癌痛患者随机分为治疗组和对照组,每组40例。对照组予三阶梯止痛法(中度疼痛予盐酸布桂嗪,重度疼痛予吗啡);治疗组在三阶梯止痛法基础上,采用离子导入仪中频导入癌痛消。两组疗程均为7天,观察镇痛效果、镇痛药使用剂量以及卡氏评分变化情况,并评价不良反应。结果治疗组较对照组起效快,止痛作用持续时间长(P<0.05),且盐酸布桂嗪片及吗啡片每日平均维持量均较对照组小(P<0.05)。两组治疗后卡氏评分均显著提高(P<0.05),且治疗组较对照组提高更为明显(P<0.05)。两组不良反应多为阿片类药物常见的不良反应,治疗组不良反应发生率为25.0%,明显低于对照组的57.5%(P<0.05)。结论中频导入癌痛消结合三阶梯止痛法治疗癌痛具有临床效果显著、起效快、止痛持续时间长、不良反应发生率低等特点,且可减少止痛药用量,从而有利于降低患者对阿片类药物的依赖性及成瘾性,提高患者的生活质量。
Objective To observe the clinical efficacy and safety of combined " Ai-tong-xiao " and " Pain relieving" method in treating cancer pain. Methods Eighty cases were randomized into treatment group and control group, 40 eases in each group. Patients in control group were treated by three steps "Pain relieving" method (Bucinnazine Hydrochloride for mild pain, Morphine tbr sever pain) , and patients in treatment group were treated by three steps "Pain relieving" method additionally. With the course of 7 days, the pain, analgesic dosage and KPS score were observed and the adverse effect was evaluated. Results Compared with the control group, treatment group worked faster, and had analgesic effect of long duration (P 〈 0.05 ). The average maintenance dose of Bucinnazine Hydrochloride and Morphine in treatment group is less than control group ( P 〈 0.05 ). The scores of KPS were improved in both of the groups ( P 〈 0. 05 ) , and it is higher in treatment group than that of the control group ( P 〈 0.05 ). The adverse effects were mostly caused by opiolds. The occurrenee rate was 25.0% in treatment group, and was 57.5% in control group, with significant difference between the two groups (P 〈 0.05). Conclusion Combined "Aitong-xiao" and three steps "Pain relieving" method has positive clinical effect with rapid action, long duration of pain relieving, low incidence of adverse reactions. It can reduce the paregoric dose to decrease the opioids dependency and addiction, improve the quality of life.
