磁共振成像及尿路造影对输尿管疾病的诊断价值

The value of MRI and MRU in diagnosis of ureter diseases

目的探讨常规磁共振成像(MRI)及磁共振尿路造影(MRU)对输尿管病变的诊断价值。方法对38例临床怀疑输尿管疾病的患者行常规MRI及MRU检查。所有病例均经手术、病理或输尿管镜检查确诊先天性畸形5例、良性狭窄7例、输尿管结石12例、输尿管癌6例和外在病变压迫8例。结果 MRU对输尿管病变的定位诊断准确率为100%(38/38),定性诊断准确率为81.6%(31/38);结合常规MRI,定性诊断准确率提高到92.1%(35/38)。结论 MRU对输尿管病变的诊断具有较高的价值,特别适用于静脉肾盂造影禁忌证和肾功能丧失者;与常规MRI结合,可提高输尿管病变的定性诊断准确率。

Objective To evaluate the diagnostic value of conventional magnetic resonance irnaging（MRI） and magnetic resonance urography（MRU） in ureter diseases. Methods Thirty-eight patients with clinically suspected ureter diseases were examed by MRI and MRU. The patients were confirmed by ureteroscopy, surgery and pathology to have the diagnosises of congenital malformation in 5 cases, benign stricture in 7 cases, uretal calculus in 12 cases, uretal carcinomas in 6 cases and extrinsic lesions in 8 cases. Results With MRU,the accuracy rate of localization diagnosis was 100% and that of qualitative diagnosis was 81.6% （31/38）, which was increased to 92.1% （35/38） when combined use of conventional MRI and MRU. Conclusion MRU is highly valuable in the diagnosis of ureter diseases, especially in the patients contraindicated to intravenous pyelography or with renal dysfunction. The accuracy rate of qualitative diagnosis can be improved by combined use of conventional MRI and MRU.
