摘要
目的 DEK蛋白与病毒感染、肿瘤、自身免疫性疾病有关。DEK蛋白的表达与胃癌的相关性方面的研究尚未见报道文中利用胃癌组织芯片研究DEK蛋白在胃癌中的表达与病理生物学行为的相关性,为胃癌的诊断及预后判定提供新的指标。方法选取121例胃癌和39例正常胃黏膜组织制作成组织芯片,利用免疫组织化学方法检测DEK蛋白在胃癌及正常胃组织中的表达水平。结果 DEK蛋白在121例胃癌中86例呈阳性表达。早期胃癌及进展期胃癌的阳性表达率与正常胃黏膜组织比较差异有统计学意义(P<0.001),进展期胃癌的阳性表达率与早期胃癌相比差异有统计学意义(P<0.001)。不同组织类型中,DEK蛋白在未分化癌、鳞状细胞癌和管状腺癌中的阳性表达率分别为66.7%、66.7%和87.5%,与黏液腺癌和印戒细胞癌中的阳性表达率0%和6.7%相比差异有统计学意义(P<0.001),管状腺癌的阳性表达率与未分化癌和鳞状细胞癌相比均有显著性差异(P<0.001)。DEK蛋白的表达与胃癌的病理学行为相关性研究结果表明,DEK蛋白在90例男性和31例女性胃癌中的阳性表达率分别为72.2%和67.7%,2组间无统计学意义(P>0.05)。Ⅳ级与Ⅰ级比较,2组间差异有统计学意义(P<0.05)。在14例浸润至肌层和58例浸润至浆膜层及浆膜外的胃癌中DEK蛋白的阳性表达率分别为78.6%和81.0%,与49例早期胃癌中的57.1%阳性表达率相比差异有统计学意义(P<0.001)。DEK蛋白在有淋巴结转移的92例和无淋巴结转移的29例胃癌中阳性表达分别为82.6%和41.4%,2组间差异有统计学意义(P<0.001)。DEK蛋白在有远处转移的44例和无远处转移的77例胃癌中的阳性表达率分别为79.5%和66.2%,2组间差异有统计学意义(P<0.05)。结论 DEK蛋白的高表达在胃癌的发生发展中起重要作用,并与胃癌的组织学类型、浸润程度、淋巴结转移及远处转移有密切相关,可作为胃癌早期诊断及预后判定的一个指标。
Objective The DEK protein is involved in viral infection, tumors and autoimmune diseases, but hitherto no report has been seen on its correlation with gastric cancer. The authors explored the correlation of the expression and pathobiological char- acteristics of DEK in gastric cancer using gastric cancer tissue microarray to provide a new biomarker for the diagnosis and prognosis of the disease. Methods We made tissue chips from 121 specimens of gastric cancer and 39 of normal mueosa gastric tissues, and determined the expression of the DEK protein in the two groups of tissues by immunohistochemistry. Results The positive expression of the DEK protein was found in 68.9% (86/121) of gastric cancer cases, with significant differences from the normal control (5.1%) (P 〈0. 001 ) as well as between the early and advanced carcinoma groups (P 〈0.001 ). The rates of positive DEK expression were 66.7% , 69.7% , 71.4% and 80.0% in the tissues of histopathologically grades I , II , III and IV, respectively, with remarkable differences between I and IV ( P 〈 0.05 ) , and 57.1% in the 49 cases of early gastric cancer, significantly lower than 78.6% in the 14 cases with muscular layer invasion and 81.0% in the 58 cases with serosa and peripheral invasion ( P 〈 0.001 ). The positive rates were 82.6% and 41.4% in the 92 cases with and 29 without lymph node metastasis ( P 〈 0. 001 ), and 79. 5% and 66.2% in the 44 with and 77 without distant metastasis ( P 〈 0.05 ). Conclusion High expression of the DEK protein plays an important role in the development and progression of gastric cancer, which is closely correlated with the histologic type, infiltration degree, lymph node metastasis and distant metastasis of the disease, and can be used as a novel marker in the early diagnosis and prognosis of gastric carcinomas.
