透明质酸对兔带软骨膜肋软骨自体异位移植后的影响

摘要 目的:观察透明质酸(HA)对兔带软骨膜肋软骨自体异位移植后的增殖及代谢的作用情况的影响。方法:健康新西兰大白兔12只,雌雄不限。直接将自体带软骨膜7、8肋软骨取出,分别移植于椎旁肌肉左右侧。左侧为实验组,右侧为对照组。实验组于术后第7天开始向移植处注射透明质酸钠注射液(5天一次),对照组则注入生理盐水。于手术后第8周,再取兔胸部对例肋软骨作为空白对照组共同进行检测。3组标本行大体形态观察,HE染色观察软骨细胞形态及增殖情况,另检测胶原蛋白含量进行比较。结果:HA实验组大体形态与空白手术对照组更为接近,NS对照组软骨在色泽和质地上均有退行性变化。HE染色结果显示HA实验组软骨细胞增生活跃,无明显钙化灶及基质分离现象;NS对照组软骨细胞呈退行性变,有大量散在的坏死及钙化灶。HA实验组胶原蛋白含量与空白手术对照组差别无统计学意义(P>0.05),HA实验组胶原蛋白含量高于NS对照组差别有统计学意义(P<0.05),空白手术对照组胶原蛋白含量高于NS实验对照组差别有统计学意义(P<0.05)。结论:HA能促进兔带软骨膜肋软骨自体异位移植后的软骨及软骨膜细胞的增殖与代谢,减少兔带软骨膜肋软骨自体异位移植后的吸收与钙化。 Objective To observe the influence of proliferation an^l metaboJism of HA's effect on the rabbit's Perichondrium Costicartilage autologous transplantation. Methods in this research, we randomly selected 12 New Zealand Rabbits regard.bess of male or female. Firstly, we directly take out the 7, 8 costal cartilage of the Rabbits and then were transplanted into vertebral side muscle left and right.The left presented the experimental group and the right presented the control group.The experimental group were injected into the graft distract with Sodium Hyaluronate on the seventh day after surgery (once every five days),wh!le the control group were injected with Physiological saline. After the eighth week,the contralateral costal cartilages of the Rabbits were taken out as the blank control and were detected together. The three groups were firstly observed morphologically generally and then HE was performed to detect the morphology and proliferation of the cartilage cells, at the same time,the level of collagen protein was detected. Results the general morphology of the HA group and the blank group was closely approximated while the NS group was degenerative on the color and texture.HE demonstrated that HA group displayed active cartilage cells, no calcifications and the matrix separation phenomenon; while the NS group performed the degeneration of cartilage cells with more scatter necrosis and calcifications.The levels of collagen protein between HA group and NS group had no statistical significance (P〉0.05), the levels of collagen protein were more then NS group (P〈0.05),and the blank group were also contained more collagen proteins then NS group (P〈0.05). Conclusion HA may promote the proliferation and metabolism of chondrocyte,while decrease the absorption and calcification after autoheterotopic transplantation of the rabbit's Perichondrium Costicartilaae.
