奥氮平合并劳拉西泮治疗广泛性焦虑疗效分析

Curative effect analysis of olanzapine combined with lorazepam in treatment of generalized anxiety disorder

目的:分析奥氮平合并劳拉西泮治疗广泛性焦虑的疗效和安全性。方法:随机将96例广泛性焦虑分为奥氮平合并劳拉西泮治疗组52例和单用劳拉西泮对照组44例。两组在性别、年龄和病程上均无显著性差异(P>0.05)。在治疗前和治疗第2、6周末采用汉密尔顿焦虑量表(HAMA)及副反应量表(TESS)评定疗效及不良反应。结果:两组2、6周末的HAMA总分和因子分都有明显降低,随着疗程延长各降分更为明显。两组间比较,治疗组6周末HAMA总分和躯体焦虑因子分降分明显大于对照组(P<0.05),显效率明显高于对照组P<0.05)。治疗组奥氮平以小剂量为主,劳拉西泮用量低于对照组(P<0.01),不良反应则明显轻于对照组。结论:小剂量奥氮平合并劳拉西泮治疗广泛性焦虑具有增效及减轻药物不良反应的特点。

Objective ： To analyze efficacy and safety of olanzapine combined with lorazepam in treatment of generalized anxiety disorder. Methods ：96 patients with generalized anxiety disorder were randomly divided into olanzapine combined with lorazepam treat- ment group （52 cases） and control group （44 cases, treated only with lorazepam）. The two groups were not significantly different in gender, age and disease duration （P〉0.05）. Before and 2 and 6 weeks after the treatment, Hamilton anxiety scale （HAMA） and treatment emergent symptom scale （TESS） were used to assess the efficacy and adverse reactions. Results： 2 and 6 weeks after the treatment, the HAMA total scores and factor scores of the two groups were significantly reduced, and the situation was more obvious as the course was prolonged. The decrease of the HAMA total score and somatic anxiety factor score of olanzapine combined with loraze- pare treatment group was higher than that of control group 6 weeks after the treatment, and the markedly effective rate was also higher than that of control group （P〈0.05）. Due to that treatment group used a small dose olanzapine as the main drug, the lorazepam dose was lower than that of the control group （P〈0.01）. Moreover, the adverse reactions were significantly lighter than that of control group. Conclusions： A small dose olanzapine combined with lorazepam in the treatment of generalized anxiety disorder, has the advan- tages of high efficiency and little adverse reactions.
