分娩球运动待产结合自主式屏气法在促进自然分娩中的临床应用

Clinical application of labor ball movement combined with independent breathholding method on improvement of natural childbirth

目的探讨分娩球运动待产结合自主式屏气法对促进自然分娩的效果。方法将530例初产妇按产妇意愿分为观察组278例和对照组252例,观察组产妇第一产程在助产士指导下使用分娩球自由体位运动待产,第二产程采用自主式屏气法;对照组产妇第一产程采用传统仰卧位分娩和助产士适时指导,第二产程每次宫缩开始时,助产士指导产妇屏气用力。结果观察组278例中241例自然分娩,对照组252例中195例自然分娩,两组比较,x2=7.889,P<0.01;观察组自然分娩产妇产程进展快、屏气用力时间短、产后出血量少、新生儿窒息率低、医疗干预少、疼痛程度轻,与对照组比较,差异有统计学意义。结论分娩球运动待产结合自主式屏气能提高自然分娩率,改善母婴预后。

Objective To study the effect of labor ball movement combined with independent breathholding method on improvement of natural delivery. Method Divide 530 primiparas into observation group （n=278） and control group （n= 252） according to their intention. Primiparas in observation group use birth ball in free labor position in first stage of labor and independent breathholding method in second stage of labor guided by midwives. Primiparas in control group are in routine horizontal position and guided by midwives when necessary in first stage of labor. Then hold breath and exert strength under guidance of midwives every time uterine contraction starts in second stage of labor. Result 241 from 278 primiparas in observation group undergoing natural childbirth and 195 from 252 primiparas in control group undergoing natural child- birth, x2 = 7. 889, P〈0.01. Natural delivery in observation group has shorter labor stage, shorter holding breath and exerting strength time, less blood loss, lower neonatal asphyxia rate, less medical intervention and less pain compared to control group. The difference is significant. Conclusion Labor ball movement combined with independent breathholding method can improve rate of natural delivery and maternal--fetal prognosis.
