ELF方案联合奥沙利铂治疗晚期胃癌的疗效观察

The clinical evaluation of ELF regimen combined with oxaliplatin in the treatment of advanced gastric cancer

目的观察依托泊苷、亚叶酸钙、5氟尿嘧啶(ELF方案)联合奥沙利铂治疗晚期胃癌的临床疗效,探讨其临床应用价值。方法选取2010年2月至2011年9月收治的64例晚期胃癌患者,随机分为对照组(32例)和观察组(32例)。对照组患者单纯采用ELF方案进行治疗,观察组患者在此基础上联合奥沙利铂进行治疗,3周为1个疗程,共进行2个疗程治疗。观察并比较两组患者的临床疗效及不良反应发生情况。结果观察组患者总有效率为50.0%(16/32),明显高于对照组的28.1%(9/32),两组比较差异有统计学意义(P<0.05)。观察组患者不良反应较轻,主要为骨髓抑制(8例)、神经感觉异常(8例)及胃肠道反应(6例),明显低于对照组的17例、15例和18例,两组比较差异有统计学意义(P<0.05)。结论 ELF方案联合奥沙利铂治疗晚期胃癌疗效显著,不良反应小,可作为临床治疗晚期胃癌的首选方案,值得推广和应用。

Objective To evaluate the efficacy of the combination of ELF（ VP16/CF/5-Fu） and oxaliplatin in the treatment of advanced gastric cancer,and investigate the clinical application value. Methods 64 cases of advanced gastric cancer were evenly divided into two groups. The control group（ n = 32） was treated by ELF scheme. On the basis,the observation group（ n = 32） was treated by oxaliplatin. After treatment 6 weeks,clinical efficacy and adverse reaction were observed. Results The total effective rate of the observation group was 50. 0%（ 16 /32）,was significantly higher than it of the control groups＇ 28. 1%（ 9 /32）,there was significant difference between two groups（ P〈0. 05）. In the observation group, 8 cases happened bone marrow suppress,8 cases happened unusual feeling and 6 cases happened gastrointestinal reaction. The adverse reaction of the observation group was obviously lighter than it of the control group,there was significant difference between two groups（ P〈0. 05）. Conclusions ELF regimen combined with oxaliplatin in the treatment of advanced gastric cancer has accurate clinical curative effect and smallside-effect and is worth clinical spreading.
