摘要
目的探讨乙肝患者血清标志物(HBV-M)与前S1抗原(PreS1)及HBV-DNA之间的相关性。方法采用实时荧光定量PCR对225例HBV感染者血清中HBV-DNA含量进行检测,同时运用ELISA方法检测HBV血清标志物和PreS1,按照不同HBV-M模式对PreS1和HBV-DNA进行分析。结果在受检的225例标本中,大三阳组中HBV-DNA阳性率95%,PreS1阳性率96.66%,HBV-DNA和PreS1阳性率接近,组间比较差异无统计学意义(χ2=0.00,P>0.05);小三阳组中HBV-DNA阳性率50.86%,PreS1阳性率93.10%,PreS1阳性率明显高于HBV-DNA阳性率,组间比较差异有统计学意义(χ2=51.32,P<0.01);在HBsAg阳性中HBV-DNA阳性率66.32%(126/190),PreS1阳性率94.74%(180/190),高于HBV-DNA阳性率,组间比较差异有统计学意义(χ2=48.93,P<0.01);在HBeAg阳性中HBV-DNA阳性率为93.55%(58/62),PreS1阳性率为95.16%(59/62),二者差异无统计学意义(χ2=0.00,P>0.05);在HBeAg阴性中HBV-DNA阳性率为42.33%(69/163),PreS1阳性率为76.07%(124/163),高于HBV-DNA阳性率,组间比较差异有统计学意义(χ2=38.42,P<0.01)。结论在HBeAg阳性中,HBV-DNA和PreS1有较高相关性,三者联合检测有利于全面监测乙肝病情和提高乙肝疗效。
Objective To investigate the correlations on HBV serum markers in patients with preSl-antigen(preS1-Ag) and HBV-DNA. Methods Real-time fluorescence quantitative PCR was used on 225 cases of HBV infected patients to detected HBV- DNA contents in serum, and ELISA was used for the detection of HBV serum markers and preS1-Ag, respectively. According to the different HBV-M models,preS1-Ag and HBV-DNA were analyzed. Results In the 225 subjects,the positive rate of HBV-DNA in big 3 this world group was 95 %, preS1-Ag positive rate was 96.66 %, HBV-DNA positive rate compared close to that of preS1-Ag, there was no difference between these two groups( x^2=0.00,P〉0.05) ;the positive rate of HBV-DNA in small 3 this world group was 50.86%, preSi-Ag positive rate was 93. 10%, which was higher than that of HBV-DNA, comparisons between these two groups were statistically significant(x^2=51.32, P〈0.01);the positive rate of HBV-DNA in HBsAg was 66. 32% (126/190), preS1-Ag positive rate was 94.74% (180/190),preS1-Ag positive rate was higher than the positive rate of HBV-DNA, the differ- ence between groups was significant(x^2= 48.93, P〈0.01). In HBeAg positive group, the positive rate of HBV-DNA was 93.55 % (58/62) ,preS1-Ag positive rate was 95.16% (59/62),there was no significant difference between these two groups(x^2 =0. 00,P〉 0.05). In HBeAg negative group, HBV-DNA positive rate was 42.33 % (69/163), preS1-Ag positive rate was 76.07 % (124/163), which was higher than HBV-DNA positive rate, the difference between groups was statistically significant(x^2 = 38.42, P〈0.01). Conclusion In the positive HBsAg group, HBV-DNA and preS1 have higher correlations,the joint detections of these three meth- ods is beneficial to comprehensive surveillance of hepatitis B disease and improving the therapeutic effect.
