固尔舒、珂立苏联合经鼻间歇正压通气治疗新生儿呼吸窘迫综合征的疗效比较

Efficacy Comparison of Curosurf,Calsurf Combined with NIPPV on Neonatal Respiratory Distress Syndrome

目的分析固尔舒、珂立苏联合经鼻间歇正压通气治疗新生儿呼吸窘迫综合征的疗效。方法选择2011年2月至2013年2月在海南省农垦总医院接受治疗的新生儿呼吸窘迫综合征患者作为研究对象,联合用药通气组患者给予固尔舒、珂立苏联合经鼻间歇正压通气治疗,单纯正压通气组单纯使用经鼻间歇正压通气治疗,比较两组患者的血氧指标、呼吸机指标、并发疾病情况等差异。结果联合用药通气组患儿治疗后6 h、12 h血氧分压及pH值均著显高于单纯正压通气组(P<0.05),治疗后6 h、12 h二氧化碳分压显著低于单纯正压通气组[(40.21±5.98)mm Hg vs(46.34±5.29)mm Hg]、[(38.76±5.23)mm Hg vs(44.34±5.78)mm Hg](P<0.05);气道峰压、呼气末正压、呼吸频率均显著优于单纯正压通气组[(16.42±2.75)cm H2O vs(20.86±2.18)cm H2O]、[(3.21±1.37)cm H2O vs(5.03±1.26)cm H2O]、[(56.42±7.43)次/min vs(63.24±6.39)次/min](P<0.05);总并发疾病概率显著低于单纯正压通气组[2.5%vs 10%](P<0.05)。结论固尔舒、珂立苏联合经鼻间歇正压通气可以改善提高新生儿呼吸窘迫综合征患儿的肺部功能指标,减少并发疾病。

Objective To analyze the efficacy of curosurf,calsurf combined with nasal intermittent positive pressure ventilation（ NIPPV） on neonatal respiratory distress syndrome（ NRDS）. Methods A total of 80 cases with NRDS treated by Hainan Province Reclamation General Hospital during Feb. 2011 and Feb. 2013 were selected as research objects. The combination treatment group was given NIPPV plus curosurf and calsurf,while the single NIPPV group was given only NIPPV. The blood oxygenation index,ventilator index,concurrent disease index of the two groups were compared. Results 6 h,12 h after treatment,the blood oxygen partial pressure and pH value of the combination treatment group were significantly higher than the single NIPPV group,while the CO 2 partial pressure were significantly lower than the single NIPPV group [（ 40. 21 ± 5. 98） vs（ 46. 34 ± 5. 29） ],[（ 38. 76 ± 5. 23） vs（ 44. 34 ± 5. 78） ]（ P 0. 05）. The airway peak pressure,positive end-expiratory pressure,breathing rate were significantly better than that of single NIPPV group [（ 16.42 ±2.75） cm H 2 O vs（ 20. 86 ± 2. 18） cm H 2 O,（ 3. 21 ± 1. 37） cm H 2 O vs（ 5. 03 ± 1. 26） cm H 2 O,（ 56. 42 ±7.43） vs（ 63.24 ±6. 39） ]（ P 0. 05）; complication incidence was significantly lower than the single NIPPV group[2. 5% vs 10% ]（ P 0. 05）. Conclusion Curosurf,calsurf combined with NIPPV can improve lung function indexes of NRDS,and reduce complications.
