摘要
目的探讨围术期肺癌患者Thl7细胞、NK细胞及IFN-γ、IL-17的动态变化。方法选择择期行肺癌根治术的患者（肺癌组）20例，对照组lO例，于术前（T。），术毕（T，），术后1d（T：）、3d（L）、5d（T4）检测外周血中Th17、NK细胞的水平，以及IFN-γ、IL-17的浓度。结果与对照组相比，肺癌组外周m中Thl7细胞、IL一17的表达升高；NK细胞及IFN-1的表达降低（P〈0．05）。与术前相比，肺癌患者术后1dTh-17细胞及IL-17的水平明显升高，NK细胞及IFN-γ水平明显降低；术后5dTh17细胞及IL-17的水平明显降低，IFN-1水平明显升高（P〈0．05）。相关性分析结果显示，肺癌患者术前外周血中Thl7与IL-17A呈正相关性（P〈0．01）；NK与IFN-γ呈正相关性（P〈0．01）；Thl7与NK、IFN-γ均呈负相关性（P〈0．05）。结论围术期肺癌患者的免疫状态呈现先抑制后激活的状态，检测肺癌患者外周血中Th17细胞、NK细胞及IFN-γ、IL-17的表达对评估肺癌患者细胞免疫功能及疾病的治疗具有重要意义。
Objective To investigate the dynamic changes of Th17, NK and IFN -γ,IL- 17 in periop- erative patients with lung cancer. Methods Twenty cases of patients with lung cancer surgery were chosen ran- domly. Ten cases of healthy were used as control. We detected the express of Thl7, NK, IFN -γ and IL - 17. Re- suits Compared with the control group, the levels of Thl7 and IL - 17 increased significantly in lung cancer group（ P 〈 0.05 ） ;Compared with the pre -operation ,the levels of Thl7 and IL- 17 increased significantly ＇after I st day of lung cancer surgery. However, the leveJs of NK and IFN - T decreased significantly. The expression of Thl7 and IL - 17 decreased obviously after 5 days of lung cancer surgery. However, the levels of IFN -γ in- creased significantly（ P 〈 0.05 ）. The correlation analysis showed that there was a positive correlation between Th17 and IL- 17A,NK and IFN -γ of peripheral blood with lung cancer patients （P 〈 0.01 ）, negative correla- tion between Thl7 and NK, IFN -γ of peripheral blood with lung cancer patients （P 〈 0.05 ）. Conclusion The immune suppress before activating at perioperative,it is important for lung cancer patients to detect the levels of Th17 ,IL - 17 ,NK,IFN-γ in peripheral blood.
