老年高血压患者血清尿酸水平的临床观察

目的探讨老年高血压患者血清尿酸水平与高血压和高血压前期的关系。方法将2010年3月-2012年9月来社区卫服中心体检者按血压状况分为高血压组、前期组和对照组,分别比较三组尿酸水平,并对不同尿酸水平与高血压的关系进行Logistic回归分析。结果随着尿酸水平的提高,1、2、3、4水平的调整前收缩压分别为(114.2±15.8)mmHg、(126.5±32.5)mmHg、(129.3±65.2)mmHg、(133.2±51.2)mmHg,舒张压分别为(75.8±11.1)mmHg、(79.5±10.5)mmHg、(84.2±20.5)mmHg、(85.3±13.6)mmHg,均呈现上升趋势,差异具有统计学意义(P<0.05);调整混杂因素后,本趋势仍然存在(P<0.05)。对不同尿酸水平与高血压关系的Logistic回归分析可以看到,1、2、3、4级尿酸水平的高血压OR值分别为1、2.875、5.762、19.842,随血清尿酸水平的增加而增加(P<0.05);对年龄、性别、BMI、空腹血糖、TC、TG等进行调整后,两者趋势仍然存在(OR>1,P<0.05),且最高水平尿酸组出现高血压概率为1水平的3.384倍,出现正常高值概率为1水平的1.975倍。结论血清尿酸水平与老年患者高血压和高血压前期相关联,是高血压前期和高血压的独立危险因素。

Objective To explore the relationship between serum uric acid levels and hypertension in elderly patients.Methods 267 cases were divided into 3 groups according to the blood pressure, hypertensive group, pre-hypertension group and control group, the level of uric acid was compared, and Logistic regression analysis was used for the relation between different levels of uric acid and hypertension.Results With the improvement of uric acid level, systolic pressure of thefirst, second, third and fourth level before the adjustment respectively was（114.2±15.8） mmHg,（126.5±32.5）mmHg,（129.3±65.2）mmHg,（133.2±51.2）mmHg; diastolic pressure respectively was（75.8±11.1）mmHg,（79.5±10.5）mmHg, （84.2±20.5）mmHg,（85.3±13.6）mmHg; both showed a rising trend accompany with the uric acid level rising; after adjustment for confounding factors, the trend was still existed （P〈0.05）. Logistic regression analysis on the relationship between serum uric acid levels and hypertension,OR of hypertension of thefirst, second, third and fourth level before the adjustment respectively was 1,2.875,5.762,19.842, the risk of high blood pressure increased accompany with the serum uric acid level increased （OR〉1,P〈0.05）; adjustments for age, gender, BMI, fasting blood glucose, TC, TG, the trend still existed （OR〉1,P〈0.05）, and the probability of hypertension and in highest level of uric acid group was 3.384 times to the lowest group; the probability of high normal was 1.975 times.Conclusion Serum uric acid level are independent risk factors of pre-hypertension and hypertension.
