大鼠异位心脏移植术围手术期并发症的原因及预防

Reasons and Prevention of Perioperative Complications of Rat Heterotopic Cardiac Transplantation

目的 探讨大鼠异位心脏移植术围手术期并发症发生的原因及预防措施。 方法 采用改良 Ono术式对 10 8只大鼠施行异位心脏移植术 ,其中预实验 30只 ,正式实验 78只 ,手术成功率分别为 47%和 92 %。 结果 预实验者主要并发症发生率为 5 3% ,即麻醉意外、术中出血、静脉回流障碍、截瘫、供者心脏复跳失败、术后早期死亡。正式实验者并发症发生率为 8% ,且供者心脏冷、热缺血时间、心脏复跳时间较预实验者显著缩短 (P<0 .0 5 )。含有速尿的 St.Thomas液比平衡液对供者心脏有更佳的保护效果 (P<0 .0 5 )。 结论 针对围手术期并发症发生的原因进行预防 ,可使改良 Ono术式达到较高的。

Objective To study the reasons and prevention of perioperative complications of rat heterotopic cardiac transplantation. Methods One hundred and eight rats of heterotopic cardiac transplantation were performed using modified Ono procedure. Thirty rats of preliminary experiment and 78 rats of formal experiment were involved in, with 47% and 92% of successful rate respectively. Results The rate of main complications in preliminary experiment rats was 53%, including anesthetic accident, bleeding during operation, disturbance of venous return, paraplegia, rebeat failure and early death after operation. Rate of complications in formal experiment rats was 8%. Time of hot ischemia, cold ischemia and rebeat was all significantly shorter in formal experiment rats than in preliminary experiment rats ( P <0 05). Cold St.Thomas Hospital cardioplegic solution with furosemide could improve the preservation effect of rat heart. Conclusion To prevent a series of perioperative complications according to their reasons, we can acquire stable and high successful rate.
