完全腹膜外与经腹腔腹膜后腹腔镜精索静脉高位结扎术的对比研究

Comparative study of totally extraperitoneal with transabdominal retroperitoneal laparoscopic high ligation of spermatic vein

目的:对比评价腹腔镜完全腹膜外精索静脉高位结扎术及腹腔镜经腹腔腹膜后精索静脉高位结扎术治疗精索静脉曲张的应用价值。方法:回顾分析53例精索静脉曲张患者的临床资料,其中25例行腹腔镜完全腹膜外精索静脉高位结扎术(A组),28例行腹腔镜经腹腔腹膜后精索静脉高位结扎术(B组),对比两组手术时间、肠蠕动恢复时间、最早下床时间、住院时间。结果:A、B两组肠蠕动恢复时间及最早下床时间分别为(5±2.5)h、(10±2.6)h及(7±1.4)h、(12±2.1)h,差异有统计学意义(P<0.05);手术时间、住院时间两组相比差异无统计学意义。结论:腹腔镜完全腹膜外精索静脉结扎术治疗精索静脉曲张对腹腔干扰小、术后康复快,是较理想的治疗方法。

Objective ：To compare and evaluate the values of clinical application of totally extraperitoneal laparoscopic high ligation of spermatic vein and transabdomiuaI retroperitoneal laparoscopic high ligation of spermatic vein in the treatment of varicocele. Methods：The clinical data of 53 patients with varicocele were retrospectively analyzed. 53 patients were divided into group A （25 patients who underwent totally extraperitoneal laparoseopie high ligation） and group B （28 patients who underwent transabdomiual retroperitoneal laparoscopic high ligation）. The operation time,time of gastrointestinal function recovery, postoperative ambulation time and hospital stays were compared between the 2 groups. Results：The time of gastrointestinal function recovery and postoperative ambulation time in group A and group B were respectively （5 ±2.5） h,（10 ±2.6） h and （7 ±1.4） h,（12 ±2.1） h （P 〈0.05）. But the operation time and hospital stay showed no significant differences. Conclusions：Compared with laparoseopie transabdominal operation, totally extraperitoneal laparoscopic high ligation of spermatic vein has the advantage of little abdominal interference and quick recovery. It is a better treatment for varicocele.
