可吸收螺钉治疗Pipkin骨折的临床体会

摘要 目的探讨应用可吸收螺钉治疗Pipkin骨折的临床疗效。方法 1997年5月至2012年2月,笔者采用可吸收螺钉内固定治疗不同类型的Pipkin骨折10例。其中男8例,女2例;18~57岁,平均39.5岁;左侧6例,右侧4例。按照Pipkin分型:Ⅰ型2例,Ⅱ型4例,Ⅲ型1例,Ⅳ型3例。对合并髋臼、股骨颈骨折者根据骨折类型不同采用相应的内固定治疗。观察记录手术时间、术中出血量、并发症等情况。术后随访按照Thompson-Epstein评分法中临床和放射学评价标准评估髋关节功能。结果手术时间78~150分钟,平均104.1分钟;出血量230~650ml,平均349.9ml。8例获得良好随访,2例术后失访,随访时间8~132个月,平均48.3个月。术后1例发生股骨头无菌性坏死,于2年后行人工髋关节置换术。1例出现坐骨神经损伤症状,经对症处理后康复。无切口感染、骨折固定失败等并发症发。按照Thompson-Epstein评价标准,优4例,良2例,可1例,差1例。结论可吸收钉是治疗股骨头骨折较理想的内固定物。Pipkin骨折应尽早行闭合复位、牵引,根据创伤控制原理尽早切开解剖复位、可吸收钉固定,术后早活动、晚负重功能锻炼,能取得较为满意的疗效。
