摘要
目的:比较钉合与缝合技术应用于骨科肩肘部位手术切口中的疗效观察,探讨1次性皮肤缝合器在骨科骨科肩肘部位皮肤切口中的应用价值。方法:选择150例骨科肩肘部位手术病人,随机分为2组,治疗组77例,术中采用1次性皮肤缝合器缝合切口,对照组73例采用常规丝线缝合切口。分别对2种缝合方法在切口感染、切口裂开、炎性反应(红肿)、分泌物、坏死、过敏反应、术后3个月切口"蜈蚣症"的发生率,缝合速度,拆线(钉)时的疼痛程度,9项指标进行统计学分析。结果:2组在缝合速度,拆线(钉)时的疼痛程度,术后3个月切口"蜈蚣症"的发生率方面差异显著(P<0.01),治疗组在拆钉时比对照组拆钉时有较高的疼痛程度;但有更快的缝合速度;术后3个月随访,切口瘢痕"蜈蚣症"的发生率为0%,对照组为35.61%;2组在切口感染、切口裂开、炎性反应(红肿)、分泌物、坏死、过敏反应方面无显著差异。结论:钉合技术(1次性皮肤缝合器)相对于缝合技术(常规丝线)在缝合骨科肩肘部位皮肤切口有更快的缝合速度及更好的美容效果,有较大的应用价值。
Objective: To compare stapling and suture techniques used in orthopedic incision in the shoulder and elbow parts efficacy, and to explore the application value of disposable skin closure in the shoulder and elbow parts of the skin incision. Methods :150 cases of patients with shoulder and elbow parts of the surgery, were randomly divided into treatment group 77 cases using a disposable skin stapler, 73 eases of the control group using conventional suture the incision. The incision infection, wound dehiscence, inflammation (swelling) , secretions, necrosis, allergic reactions, 3 months after the incision centipede disease incidence, stitching speed, stitches (nail) when the degree of pain, nine indicators were statistically analyzed. Results : The degree of pain when the two groups in the speed of stitching, stit- ches (stapled), the incision the centipede disease incidence differences were significantly (P 〈0.01 ), the treatment group have a higher degree of pain; faster suture; the centipede disease incidence was 0% after the follow - up, however, it was 35.61% in the control group; two groups were no significant differences in the incidence of infection, dehiscence ,inflammatory reaction (swelling) , secretions, necrosis, allergic reactions. Conclusion:Stapling techniques( disposable skin staplers )were faster the suture speed and better cosmetic results than suture techniques(silk) in orthopedic incision in the shoulder and elbow parts.
