右美托咪定在成人ICU镇静中的应用研究

The Application Investigation of Dexmedetomidine in Adult ICU Sedation

目的：对右美托咪定在成人ICU镇静的效果进行研究。方法：200例有创机械通气的ICU患者应用右美托咪定镇静，镇静水平控制在Ramsay评分Ⅲ-Ⅳ，以咪达唑仑作为对照，分别记录镇静起效时间、平均负荷剂量、维持剂量、机械通气时间及不良反应的发生率。结果：与咪达唑仑相比较，右美托咪定起效迅速，镇静的同时还保持“可唤醒”的睡眠状态，不影响自主呼吸功能，不延长机械通气时间和住院时间，对血流动力学影响较小。结论：患者在应用右美托咪定期间能够维持满意的镇静深度，保持“可唤醒”的睡眠状态，不影响呼吸机脱机时间，非常适合ICU环境下需要充分镇静的患者。

Objective：The study of Dexmedetomidine sedation given effect in adult ICU. Methods：200 cases of patients in ICU invasive mechanical ventilation given Dexmedetomidine sedation, sedation level control in Ramsay score III - IV stage to midazolam as a control, the onset time of sedation were recorded, with an average loading dose, maintenance dose duration of mechanical ventilation and the incidence of adverse reactions. Results： Midazolam compared Dexmedetomidine given the rapid onset of sedation while maintaining ＂to wake up＂ sleep state, does not affect respiratory function independently, does not extend the duration of mechanical ventilation and length of stay of hemodynamic learn less affected. Conclusion： Patients in the application of Dexrnedetomidine given period to maintain a satisfactory sedation, remain ＂can wake up＂ sleep state, does not affect the time of weaning ventilator ideal for ICU environments require adequate sedation of patients.
