微创血肿清除术治疗高血压性丘脑出血疗效分析

Clinical research on the minimally invasive hematoma in the treatment of hypertensive thalamic hemorrhage

目的探讨微创血肿清除术治疗高血压性丘脑出血的疗效。方法选择2010年1月—2013年1月在我院神经科进行手术治疗的高血压性丘脑出血患者90例,随机分为2组,每组45例,对照组接受传统治疗,治疗组给予微创血肿清除术治疗。分析比较2组患者的手术时间、手术出血量、住院时间、神经功能恢复情况、有效率、术后并发症等临床指标。结果与对照组相比,治疗组治疗时间、手术出血量、住院时间等显著降低(P<0.05),有效率为80.0%,显著高于对照组的53.3%(P<0.05),治疗组术后1个月后的GCS、ADL评分显著优于对照组(P<0.05),术后神经功能恢复情况也显著提高(P<0.05)。结论微创血肿清除术治疗高血压性丘脑出血疗效确切,手术微创,安全性好,术后不良反应少,值得在临床推广。

Objective To study the clinical efficiency of minimally invasive hematoma in the treatment of hypertensive thalamic hemorrhage. Methods Ninety patients with hemorrhage in de- partment of neurosurgery from 2010 January to 2013 January were randomly divided into two groups, 45 cases in each group. The control group received traditional treatment, while the treat- ment group was treated with minimally invasive surgery. The indices of operation time, operation bleeding, hospitalization time, recovery of neurological function, efficiency, postoperative complica- tions in two groups were compared. Results Compared with the control group, the operation time, bleeding volume, operation time of hospitalization in the treatment group significantly decreased （P 〈 0.05）. The effectiveness rate of the treatment group was 80 %, significantly higher than that of the control group 53.3 % （P 〈 0.05）. GCS and ADL scores of the treatment group one month after treatment were significantly better than those of the control group （P 〈 0.05 ）, and the postopera- tive recovery of neurological function in the treatment group improved significantly （ P 〈 0.05 ）. Conclusion Minimally invasive hematoma in the treatment of hypertensive thalamic hemorrhage has advantages of better curative effect, minimally invasive operation, more safety, and fewer post- operative adverse reactions. So it is worthy of promotion in clinical surgery.
