腹腔镜辅助阴式子宫切除术对中青年女性卵巢功能的影响

Effect of laparoscope-assisted vaginal hysterectomy on ovarian function in mid-aged and young women

目的探讨腹腔镜辅助阴式子宫切除术对中青年女性患者卵巢功能的近期和远期影响。方法将113例非绝经期因子宫良性病变行全子宫切除术患者随机分为腹腔镜辅助阴式子宫切除手术组(LAVH组)63例和经腹行子宫全切除术组(对照组)50例。比较2组手术时间、术中出血量、术后并发症等相关指标。结果 LAVH组手术时间、术中出血量、术后并发症、术后排气时间、住院时间、显著短于、少于对照组(P<0.05),LAVH组术后1、2年血清LH、FSH水平低于对照组,而血清P、E2水平显著高于对照组(P<0.05);LAVH组围绝经期症状发生率显著低于对照组,差异有统计学意义(P<0.05)。结论子宫全切除术对中青年患者卵巢功能有一定影响,但LAVH这种微创手术可以减少这些不利影响。

Objective To analyze the effect of laparoscope-assisted vaginal hysterectomy on ovarian function in mid-aged and young women. Methods A total of 113 cases of non- menopausal uterine benign lesions were randomly divided into two groups, laparoscope-assisted vaginal hysterectomy group （LAVH group, n = 63） and abdominal hysterectomy group （control group, n = 50）. Results LAVH group had a lower operation time, blood loss, postoperative com- plications, postoperative discharge and hospitalization time, compared with the control group pa- tients. The difference was statistically significant （P 〈 0.05）. LAVH group had a lower level of LH, FSH than the control group, one year and two years surgery （P 〈（0.05）. While serump and Fa levels were significantly higher in the LAVH group than in the control group （ P 〈 0.05 ）. LAVH group had a lower rate of perimenopausal symptoms than the control group, and the differ- ence was statistically significant （P 〈 0.05）. Conclusion There is a certain degree of influence on ovarian function in mid-aged and young patients after hysterectomy. But LAVH can reduce the ad- verse impacts than the open operation.
