大黄牡公汤煎剂保留灌肠治疗尿毒症38例临床观察及护理

Clinical Observation and Nursing Care of Uremia with Dahuang Mugong Decoction(大黄牡公汤),a Report of 38 Cases

探讨中药直肠滴注保留灌肠配合常规治疗对尿毒症的治疗效果及护理要点。选择自制的大黄牡公汤煎剂配合常规治疗尿毒症 38例 (治疗组 ) ,同时与单纯常规治疗尿毒症 38例 (对照组 )进行对照 ,观察治疗前后临床症状及血肌酐、尿素氮等参数的变化。结果 :治疗组治疗后头晕、头痛、食欲差、皮肤瘙痒等症状基本消失 ;血肌酐、尿素氮明显下降 ,总有效率为94.74% ;对照组总有效率为 71.0 5 % ;治疗组疗效明显优于对照组 (χ2 =10 .38,P<0 .0 1)。护理关键在于掌握好 4个要点 :药液的温度 ,药瓶的高度 ,输液器插入深度 ,药物滴入速度。

To explore the effect and care point of TCM retention enema and rectum dripping accompanied with routine on uremia.38 cases of uremia were treated with Dahuang Mugong Decoction (DMD) and routine.48 cases in the control group were treated with routines.The clinical symptom and blood creatinine(BCR),blood uria nitrogen(BUN) were observed.Result:In the treatment group some symptom such as headache,dizzy and skin itching disappeared,BUN and SCR decreased remarkably.The total effective rate of DMD was 94.74%,while those of control group was 71.05%.The difference between the two groups is significant(P<0.01).The 4 points of nursing care were the temperature of the decoction,the height of the decoction bottle,the depth of the infusion apparatus and the speed of decoction dribbling.
