早产儿呼吸衰竭时呼吸指数(RI)的改变和意义

The changes and significance of respiratory index in premature babies with respiratory failure

目的 :探讨早产儿呼吸衰竭时呼吸指数 (RI)的改变。方法 :对比 32例早产儿呼吸衰竭和 11例正常新生儿在呼吸指数 (RI)、肺泡 -动脉血氧差 (A- a DO2 )、动脉氧分压 (Pa O2 )、动脉二氧化碳分压 (Pa CO2 )、血氧饱和度 (Sa O2 )进行监测 ,并分析以上指标在 16例 型呼衰和 16例 型呼衰中的差异。结果 :早产儿呼吸衰竭级组 RI及血气分析结果和正常新生儿组比 ,均有显著性差异 ;同时 型呼衰组和 型呼衰组在 RI和血气分析指标上也有显著性差异。结论 :早产儿呼吸衰竭时 RI和多项血气分析均有显著性改变 ,测定 RI对临床上呼吸衰竭的分型和诊治有指导和帮助。

Objectives: To explore the changes and significance of Respiratory Index (RI) and other parameters from blood gas test in the premature babies with Respiratory Failure (RF). Method: The pH, PaO\-2, PaCO\-2, AaDO\-2, SaO\-2 and a/A were checked simultaneously and the RI was calculated from above parameters. Results: There are significance differences between the RF group and the normal control group in RI and the following parameters: pH, PaO\-2, PaCO\-2, AaDO\-2, SaO\-2. Even in premature babies with RF, there are also significance difference between the type Ⅰand type Ⅱ in RI and the above parameters. Conclusion: It's helpful to assess the RI and several parameters from blood gas test in the clinical practice of caring the premature babies with RF and to distinguish the subgroup of RF. \ \
