最大P波时限及P波离散度对预测特发性房颤的价值 被引量:5

Value of maximum P-wave duration and P-wave dispersion to the prediction of idiopathic paroxysmal atrial fibrillation

摘要 目的 探讨最大 P波时限及 P波离散度 (最大、最小 P波时限之差 )对特发性房颤的预测价值。方法 对 2 0例有特发性房颤病史者及 2 0例年龄、性别匹配的健康者 ,行 12导联同步体表心电图记录 ,测定最大 P波时限及 P波离散度。结果 在特发性房颤组 ,最大 P波时限及 P波离散度均较对照组增大 (P<0 .0 1) ,最大 P波时限 >110 ms、P波离散度 >40 ms以及两者相结合对特发性房颤预测的敏感性和特异性分别为 90 %、85 %和 75 %及 80 %、85 %和 90 %。结论 最大 P波时限及 P波离散度可用于预测特发性房颤。 Objective To explore the value of maximun P wave duration (P max ) and P wave dispersion (P d),that is,the difference between the maximum and minimun P wave duration in patients with idiopathic paroxysmal atrial fibrillation.Methods P max and P d were measured from 12 lead surface electrocardiogram among 20 patients with a history of idiopathic paroxysmal atrial fibrillation and 20 age matched healthy control subjects.Results P max and P d were found to be significantly higher in patients with a history of idiopathic paroxysmal atrial fibrillation than in the control subjects(P<0.01).A P max value of more than 110 mesc,a P d value of more than 40 mesc and combination of them separated patients from centrol subjeits, with a sensitivity of 90%,85% and 75% and a specificity of 80%,85% and 90%. Conclusion P max and P d can be used for the prediction of idiopathic paroxysmal atrial fibrillation.
