摘要
目的分析血管免疫母细胞T细胞淋巴瘤(angioimmunoblastic T-cell lymphoma,AITL)18 F-FDG PET/CT的影像学表现及分布规律,探讨其在临床分期中的价值。方法回顾性分析22例AITL患者的18 F-FDG PET/CT显像结果,评价其在临床分期中的价值。结果 (1)AITL结内侵犯的18 F-FDG PET/CT影像学表现:①全身弥漫性分布:左右对称性分布9例(40.9%),左右不对称性分布2例(9.1%);②多部位散在分布:与淋巴链走行有关4例(18.2%),与淋巴链走行无关5例(22.7%);③单部位局限性分布2例(9.1%);(2)AITL结外侵犯的18 F-FDG PET/CT影像学表现:鼻咽部侵犯最常见有10例,其余依次为口咽8例,脾脏8例,骨骼及骨髓5例,胃肠道4例,肺、胸膜及皮肤各2例,乳腺及颅内侵犯各1例;(3)在AITL分期中的价值:PET/CT显像改变了4例(18.2%,4/22)患者的分期,均为分期上调。结论血管免疫母细胞T细胞淋巴瘤呈18F-FDG高摄取;其在病灶检测及分期方面,18F-FDG PET/CT显像有优势。
Objective To analyse the imaging findings and distribution of ^18F-FDG PET/CT in angioimmunoblastic T cell lymphoma (AITL), and to evaluate the value of ^18F-FDG PET/CT in the clinical staging. Methods We retrospective ly analysed ^18 F-FDG PET/CT imaging findings of 22 patients with AITL, and evaluated its value in the clinical staging. Results ()1The imaging findings and distribution of ^18F-FDG PET/CT in AITL with nodal involvement. ① Whole-body multiple lesions with diffuse distribution: symmetrical distribution was observed in 9 of 22 patients (40.9%), and asym metrical distribution was observed in 2 of 22 patients (9.1 %) ; ② Multi-lesions in multi-locations with scattering distribu tion: lesions traveling along the lymphatic chain were observed in 4 of 22 patients (18.2%), and lesions without traveling along the lymphatic chain were observed in 5 of 22 patients (22.70/00) ; ③ Limited distribution was observed in 2 of 22 pa tients (9.1 %). (2) The imaging findings of JSF-FDG PET/CT in AITL with extranodal involvement. The more frequent sites of the lymphoma lesions were nasopharynx (10/22), oropharynx (8/22), spleen (8/22), bone and bone marrow (5/ 22), gastrointestinal (4/22), lung (2/22), pleura (2/22), skin (2/22), breast (1/22), brain (1/22). (3) The value of 18 F-FDG PET/CT imaging in the clinical staging of AITL. The stage of disease was changed in four patients on the basis of 18F-FDG PET/CT. All patients were upstaged. Conclusion The lesions of AITL are ^18F-FDG avid, and PET/CT seems to be useful in detecting and staging of this disease.
