摘要
【目的】探讨沐舒坦静脉推注联合雾化吸入治疗脑卒中伴坠积性肺炎的临床疗效。【方法】选取2012年5月至2013年5月本院收治的脑卒中伴坠积性肺炎患者80例,按照随机数字表法分为观察组和对照组各40例。对照组给予抗生素、对症、治疗原发病等常规治疗。观察组在常规治疗的基础上给予沐舒坦静脉推注联合雾化吸入。观察并对比两组的临床疗效,肺部哆音和胸片阴影消失时间,治疗前后的动脉血二氧化碳分压(PaCO2)和氧饱和度(SaO2)的变化。【结果】观察组临床有效率明显高于对照组(85%VS62.5%),且两组相比较差异有显著性(P〈0.05);观察组肺部哆音和胸片阴影消失时间明显短于对照组(P〈0.01)。治疗前两组的SaO2和PaCO2比较差异无显著性(P〉0.05),观察组治疗后的SaO2和PaCO2较治疗前明显改善(P〈0.05),且明显优于对照组(P〈0.05),而对照组治疗后的SaO2和PaCO2与治疗前比较差异无显著性(P〉0.05)。【结论】沐舒坦静脉推注联合雾化吸入治疗脑卒中伴坠积性肺炎的疗效显著,值得临床推广。
[Objective]To explore the clinical efficacy of intravenous injection combined with atomization inhalation of mucosolvan for the treatment of cerebral apoplexy with hypostatic pneumonia. [Methods] A total of 80 patients with cerebral stroke and hypostatic pneumonia in our hospital from May 2012 to May 2013 were selected. According to random number table method, all patients were divided into observation group and control group with 40 patients in each group. The control group was given conventional treatment such as antibiotics, symptomatic and primary disease treatment. The observation group was given intravenous injection combined with atomization inhalation of mucosolvan based on conventional treatment. Clinical efficacy, the disappearance time of lung rale and chest X-ray shadow, arterial blood CO2 partial pressure(PaCO2 ) and oxygen saturation (SaO2) before and after treatment of two groups were observed and compared. [Results] The clinical effective rate of observation group was obviously higher than that of control group (85 % vs. 62.5%), and there was significant difference between two groups( P 〈0.05). The disappearance time of lung rale and chest X ray shadow of observation group were obviously shorter than those of control group( P 〈0.01). There was no significant difference in SaO2 and PaCO2 before treatment between two groups( P 〉0.05). Compared with before treatment, SaO2 and PaCO2 of observation group after treatment were obviously improved( P 〈0.05) and obviously better than those of control group ( P 〈0.05). There was no significant difference in SaO2 and PaCO2 of control group between before and after treatment( P 〉0.05). [Conclusion]Intravenous injection combined with atomization inhalation of mucosolvan for the treatment of cerebral stroke with hypostatic pneumonia has significant efficacy, so it is worthy of clinical pro motion.
