一次成形耳廓再造术在适宜患者群中的应用研究

摘要 目的探讨“一次成形”耳廓再造术在适宜患者群中的应用。方法选取自2011年3月至2013年7月期间本科室收入的24例需外耳再造患者,应用自体肋软骨支架移植行一次成形法耳廓再造。对照组为同期科室收入的相同病例,应用扩张皮瓣法行耳廓再造。结果经过术后1个月~2年的随访,平均随访约1年,其中拥有耳甲腔的再造耳形态逼真。Ⅱ度小耳畸形患者,与扩张皮瓣法耳廓再造相比,一次成形法耳甲腔较浅。24只再造耳廓总体形态效果较为满意。结论“一次成形”技术操作周期短,并发症少,与扩张皮瓣法相较,亦可以显现耳廓表面细微结构,兼具外形与立体感,在合适的患者群中,是一项可靠稳定的外耳再造术。 Objective To analyze the clinical efficacy of one-stage total process technique in ear reconstruction on eligi- ble patients. Methods 24 patients from March 2011 to July 2013 were selected to undertake the one-stage total ear reconstruc- tion using autologous costal cartilage framework. The control group is made up by patients in same period using soft tissue ex- pander in ear reconstruction. Results A11 patients were followed up for 1 m ~ 2 Y(average 1 y)after surgery. For that cases that possessed cavity of auricular concha, the reconstructed ears were coincidence with the normal side on location, form and dimen- sion. While for the cases belonged to degree 1I microtia, Compared with the ear reconstruction using soft tissue expander, the one-stage total reconstructed ears seemed like having a little shallow cavity of auricular concha. However, the overall appear- ance was relatively satisfied in all 24 reconstructed auricles. Conclusions Compared with the ear reconstruction using soft tis- sue expander, the one-stage technique is suitable to eligible patients for the priority on shorter treatment time with few compli- cations and acquiring excellent reconstructed structure as well.
