茎突过长症的诊断和手术治疗5例报告

Elongated styloid process: A report 5 cases

目的总结茎突过长症的临床表现和手术方式。方法对5例茎突综合征病例进行螺旋CT或X线平片茎突检查,对其临床表现和手术方式进行分析。结果 3例患者CT检查显示患侧茎突长分别40 mm、42.8mm、84.0mm、85.6mm(1例患者为双侧患病);2例患者X线平片显示患侧茎突长度为40.0mm和37.0mm,较正常人明显增长。其中4名患者行口外或口内手术入路茎突截短术,手术效果良好。结论 CT容积漫游技术(volume rendering technique,VRT)较X线平片更能直观准确地显示茎突形态和长度。茎突截短术是茎突综合征有效的治疗方法。

Objective To summarize the clinical manifestations of elongated styloid process and surgical approach. Methods Five patients with elongated styloid process were included and examined by CT scan or X-ray. The symptoms,radiology images and surgical approach were discussed. Results The length of the styloid process in 3 patients shown on CT was 40. 0 mm,42. 8mm,84. 0mm, 85.6mm （one patients with bilateral elongated styloid process）. In other 2 patients X-ray showed that the length of elongated styloid process was 40. 0mm and 37.0mm,which were much longer than normal ones. The elongated styloid process was truncated through parapharyngeal approach in two cases and upper neck approach in two cases. One patient with slight symptom was untreated. The outcome of the treatment was satisfactory. Conclusion Surgery is the effective modality for elongated styloid process. CT volume rendering technique can show the morphology and length of styloid process more accurately than traditional X-ray.
