摘要 目的:探讨磁共振弥散加权成像(DW-MRI)鉴别腮腺肿瘤的价值。方法:对经手术病理证实并于术前行DW-MRI检查的162例腮腺肿瘤(良性肿瘤108例,恶性肿瘤54例)进行回顾性分析。在b=0,1000s/mm2的条件下,以肿瘤实质部分为感兴趣区获取各肿瘤的表观弥散系数(ADC)均值。通过统计分析比较不同肿瘤之间ADC均值的差异。结果:腮腺多形性腺瘤,腺淋巴瘤和其他良性上皮性肿瘤的ADC均值分别为1.46±0.29×10-3mm2/s,0.86±0.14×10-3mm2/s和1.15±0.36×10-3mm2/s(P<0.01);腮腺恶性上皮性肿瘤和其他恶性肿瘤(淋巴瘤和肉瘤)的ADC均值分别为0.96±0.21×10-3mm2/s和0.71±0.13×10-3mm2/s(P<0.01)。此外,腮腺良性和恶性肿瘤的ADC均值分别为1.15±0.36×10-3 mm2/s和0.88±0.22×10-3 mm2/s(P<0.01)。结论:DW-MRI能为腮腺良性肿瘤和恶性肿瘤的鉴别诊断提供有价值信息。 Purpose: The aim of this study was to determine the diagnostic value of diffusion-weighted MR imaging (DW-MRI) in differentiating benign tumors from malignant tumors of the parotid gland. Methods: Prior to surgery and pathologic verification, diffusion-weighted echo-planar MR imaging was performed on 162 subjects who suffered from parotid gland tumors (108 benign tumors and 54 malignant tumors). Mean apparent diffusion coefficient (ADC) values were calculated from diffusion-weighted MR imaging that were obtained with b factor of 0 and 1000 s/minE. The mean ADC values were obtained from the solid portion of each parotid gland tumor. Results: The mean ADC values were with significant differences (P〈0.01) among parotid pleomorphic tumors (n=47, 1.46±0.29 ×10-3mm2/s), Warthin tumors (n=47, 0.86±0.14 ×10-3mm2/s) and other benign epithelial tumors (n=14, 1,15±0.36×10-3mm2/s). Also, the mean ADC values were with significant differences (P〈0.01) between malignant epithelial tumors (n=37, 0.96±0.21×10-3mm2/s) and other malignant tumors (n=17, 12 lymphomas and 5 sarcomas, 0.71 ±0.13×10-3mm2/s). In addition, there were significant differences of mean ADC values between parotid benign tumors (1.15±0.36×10-3mm2/s) and malignant tumors (0.88±0.22×10-3mm2/s). Conclusion: DW-MRI is able to provide valuable information for the differentiation between parotid benign and malignant tumors.
