围绝经期肥胖妇女游离睾酮、性激素结合球蛋白及胰岛素抵抗的研究 被引量:1

摘要 目的:探讨围绝经期肥胖妇女游离睾酮、性激素结合球蛋白(SHBG)与胰岛素抵抗的相关性及在不同月经状态下的差异。方法:①将年龄40~50周岁肥胖妇女96例,按月经周期分为3组:规律(I组)、2次月经周期37~60d(Ⅱ组)、闭经2~12个月(III组),进行游离睾酮、SHBG、空腹血糖(FBG)及空腹胰岛素(FINS)的检测;并计算游离睾酮指数(FAI)和胰岛素抵抗指数(HOMA—IR)。同时对96例研究对象进行FAI、SHBG与HOMA—IR的相关性分析。结果:3组SHBG比较,差异无统计学意义(P〉0.05),FAI、HOMA—IRⅢ组与Ⅰ、Ⅱ组比较差异有统计学意义(P〈0.05)。96例的各项指标用Spearman相关分析:HOMA—IR与FAI呈正相关(r=0.50,P=0.00),与SHBG呈负相关(r=-0.44;P=0.00)。结论:年龄、体重、雌二醇(E2)相当时,围绝经期肥胖妇女不同的月经状态与HO—MA-IR、FAI有关,与SHBG无关。随着月经紊乱的出现,胰岛素抵抗及游离睾酮有增高的趋势。
