截骨复位植骨钢板内固定治疗桡骨远端畸形愈合的疗效分析 被引量:6

摘要 目的探讨截骨复位植骨钢板内固定治疗桡骨远端畸形愈合的疗效。方法回顾性分析2011年1月至2012年9月收治的12例桡骨远端骨折畸形愈合患者资料,男1例,女11例;年龄27—72岁,平均52.2岁。桡骨短缩6~12ram(平均9.7mm),桡骨桡侧皮质短缩9~18ram(平均14.6mm)。侧位X线片测量掌倾角度为3°~-42°(平均-17.9°)。均采用桡骨截骨、髂骨和人工骨混合植骨掌侧角钢板固定治疗,术后积极进行功能锻炼。结果所有患者术后获4~20个月(平均8.6个月)随访,术后3~4个月时均达临床愈合。患侧腕关节向桡侧、背侧移位畸形均改善。末次随访时前臂旋后平均为83.7°±0.5°,桡偏角平均为15.1°±0.1°,尺偏角平均为35.3°±0.2°,患侧疼痛视觉模拟评分平均为(1.5±0.2)分,腕关节Cooney评分平均为(81.4±0.5)分,以上项目较术前[36.2。±0.4°、28.7°±0.6°、21.6°±0.3°、(7.0±0.2)分和(20.4±0.2)分]均有改善,差异有统计学意义(P〈0.05)。手指屈伸未受限。患者均对手术结果满意。结论截骨复位植骨钢板内固定治疗桡骨远端畸形愈合可获得理想的治疗效果。
