大口径胰管空肠吻合术治疗胰管结石

Clinical application of large caliber pancreatic duct-jejunostomy in treatment of pancreatic duct stone

目的 :探讨胰管结石手术治疗效果。方法 :回顾总结胰管结石患者 14例 ,沿主胰管方向切开胰腺 ,切口长度为 6～ 10 cm,取净结石后行胰管空肠 Roux- Y吻合术。结果 :本组结石取净率为 10 0 % ,吻合口大 ,引流通畅 ,胰管减压效果明显 ,有利于再生结石排出 ,术后近期无死亡病例。腹痛完全缓解 10例 (71.4% ) ,部分缓解 4例 (2 8.6 % )。糖尿病和腹泻症状均较术前明显缓解。结论 :胰管结石确诊后应争取早日手术治疗 ,首选胰管空肠大口径 Roux-

Objective:To study theraputic effect of pancreatic duct stone extraction.Methods:For 14 cases of pancreatic duct stone admitted during 1994～1998, the pancreas along the major panceratic duct were incised for 6cm to 10cm. Roux Y pancreatic duct jejunostomy was performed following the stone extration.Results:Of all 14 cases, stone extraction is complete. Large pancreatic duct jejunostomy and good drainage lead to apparent decompression and extraction of recurrent stone. 10 patients recovered completely after the treatment and 4 patients recovered partially. Symptoms caused by diabtetes and diarrhea relieved.Conclusion:Early treatment of pancreatic duct stone by larger caliber Roux en y pancreatic duct jejunostomy is the first choice for stone extraction.
