摘要
目的 明确急性化脓性髋关节炎关节内压的变化规律并探讨其临床意义。方法 对 16例患者的 16侧病髋和 12侧健髋的关节内压进行被动活动下动态测定。结果 16侧病髋中立位关节内压高达 18 42kPa± 3 87kPa ,高于健侧的 0 2 7kPa± 0 2 6kPa(P <0 0 1) ;屈髋时关节内压进行性下降 ,屈 40°时最低 ,平均为 3 36kPa± 1 77kPa ,压力变化差异有高度显著性 (P <0 0 1)。结论 在急性化脓性髋关节炎的治疗中 ,应用牵引等强制性伸髋措施可导至持续性髋关节内高压状态 ,会对股骨头血供造成严重危害。
Objective To determine the dynamic changes of the intra articular pressure (IAP) of the acute septic arthritis of the hip and investigate its clinical significance. Methods IAP of 16 diseased hips and 12 normal hips of 16 patients with acute septic arthritis was dynamically measured at the hip joint moving from extension to flexion. Results IAP of the diseased hips at extension (0°) was 18 42 kPa±3 87 kPa, and then it dropped to 3 36 kPa±1 77 kPa at the hip joint flexed to 40°,the changes of IAP was significant ( P <0 01).IAP of the diseased hips was obviously higher than that of the control hips( P< 0 01). Conclusion The continual hyperpressure of hips at the extension position is more harmful to the blood supply of the femoral head than flexion position during treatment of the disease.
