摘要
【目的】观察肾炎康复片联合氯沙坦钾治疗慢性肾炎蛋白尿的临床疗效。【方法】将82例肾功能正常的慢性肾炎患者随机分为氯沙坦钾组和肾炎康复片联合氯沙坦钾组(联合组),分别检测2组治疗前后24 h尿蛋白定量、尿N-乙酰-β-D氨基葡萄糖甘酶(NAG酶)、血肌酐和细胞因子白细胞介素-6(IL-6)、白细胞介素-18(IL-18)的含量。【结果】治疗3个月后,联合组的总有效率为80.5%,氯沙坦钾组为61.0%,2组比较,联合组疗效优于氯沙坦钾组(P<0.05)。联合组的24 h尿蛋白、尿NAG酶显著低于氯沙坦钾组(P<0.05),血肌酐、尿素氮水平2组比较差异无统计学意义(P>0.05)。细胞因子方面,联合组的IL-6、IL-18的改善显著优于氯沙坦钾组(P<0.05)。【结论】肾炎康复片联合氯沙坦钾较单用氯沙坦钾能更有效地降低慢性肾炎患者蛋白尿,并具有免疫调节作用。
Objective To observe the therapeutic effect of Chinese herbal medicine recipe of Nephritis Rehabilitation Tablets combined with losartan potassium for chronic nephritis. Methods Eighty-two chronic nephritis patients with normal renal function were randomized into ]osartan potassium group (treated with losartan potassium alone) and combination group (treated with Nephritis Rehabilitation Tablets and losartan potassium). Before and after treatment, 24-hour urinary protein excretion, urinary n-acetyl-β-d- glucosaminidase (NAG) , serum creatinine, cytokines of interleukin-6 (IL-6) and interleukin-18 (IL-18) were measured in both groups. Results After treatment for three months, the total effective rate of the combination group was significantly higher than that of Losartan potassium group (80.5% vs 61.0%, P〈0.05). And 24 h urinary protein excretion and urinary NAG in the combination group were lower than that in losartan potassium group (P〈0.05). However, there were insignificant differences of serum creatinine and blood urea nitrogen between the two groups (P〉0.05) . The improvement of IL-6 and IL-18 in the combination group was superior to that in losartan potassium group (P〈0.05). Conclusion Nephritis rehabilitation tablets combined with ]osartan potassium shows better effect on chronic nephritis patients than losartan potassium alone, and its mechanism may be related with the reduction of proteinuria and with the regulation of immune system.
