摘要 目的:探讨CD147、MMP-2在葡萄胎组织中的表达及两者间的相互联系,预测葡萄胎恶变的可能。方法:应用免疫组织化学法对葡萄胎患者首次清宫术后40例组织标本(其中葡萄胎组31例,侵葡组6例,失访3例)进行CD147、MMP-2的检测,并与正常早孕患者绒毛组织作比较。再将葡萄胎高危组分为恶变组和未恶变组,应用免疫组织化学法检测CD147、MMP-2在含有高危因素的葡萄胎患者中的表达,分析其与葡萄胎组织恶变的关系。结果:①CD147主要表达于合体滋养层细胞以及增生滋养层细胞的胞膜,少量表达于胞浆;MMP-2主要表达于合体滋养层细胞以及增生滋养层细胞的胞浆,少量表达于胞膜。②CD147、MMP-2在侵蚀性葡萄胎组、葡萄胎组、正常绒毛组的表达呈依次递减的趋势,且两组间比较有统计学意义。③在含有高危因素的恶变组中,CD147、MMP-2平均灰度值较未恶变组的平均灰度值低(P<0.05)。④侵蚀性葡萄胎组、葡萄胎组、正常绒毛组中MMP-2与CD147的表达呈正相关。结论:①CD147、MMP-2在葡萄胎、侵蚀性葡萄胎发生发展过程中可能有促进作用,且有可能促进葡萄胎组织的恶变。②在含有高危因素的葡萄胎恶变患者中CD147、MMP-2的表达较未恶变组明显升高,提示联合检测CD147、MMP-2对预测葡萄胎的恶变具有一定的临床意义,有可能成为临床预测葡萄胎恶变的重要指标。 Objective: To explore the expressions and relationships of CD147 and MMP -2 in hydatidiform mole tissues and determine whether it can become an important index to predict the malignant change of hydatidiform mole. Methods: 40 cases were pathologically confirmed as hydatidiform mole. Immune histochemical method was applied to measure CD147, MMP -2 in the hydatidiform mole, including 31 cases of hydatidiform mole group, 6 cases of invasive hydatidiform mole, 3 eases lost to follow - up, and compared with 30 cases of patients with normal early pregnant villi tissue. Then hydatidiform mole malignant change high -risk group (33 cases) were divided into malig- nant group (6 cases), not malignant group (27 eases). Expressions of CD147 and MMP -2 and the relationships with hydatidiform mole tis- sues of malignant transformation were analyzed. Results: ①MMP - 2 was mainly expressed in syncytiotrophoblast cells, and cytotrophoblast cells, cytoplasm, a small amount of expression in syncytiotrophoblast cells, and cytotrophoblast cells, cytoplasm in cell membrane; CD147 was mainly expressed in syncytiotrophoblast cells, and cytotrophoblast cells, cytoplasm in cell membrane. ②CD147and MMP- 2 showed a decreasing trend in the expression of invasive hydatidiform mole group, hydatidiform mole group, normal villi group, and the difference between the two groups was statistically significant. ③The average gray value of CD147 and MMP - 2 in the malignant group with risk factors were less than those of without malignant group g average gray value , and the differences were statistically significant. ④ The expressions of MMP - 2 and CD147 in aggressive hydatidiform mole group, hydatidiform mole group and normal villi had certain correlation and were positively correlated. Conclusion: ①CD147, MMP -2 may have a role in the process and development of hydatidiform mole, and may promote the malignant transformation of hydatidiform mole. ②The expressions of CD147 and MMP - 2 in malignant group are obviously higher than those of without malignant group, which indicate that the combined detection of CD147 and MMP - 2 has clinical significance in predicting malignant transformation in hydatidiform mole and might become clinically important predictor of hydatidiform mole malignant change.
