摘要
目的探讨锌指蛋白36(ZFP36)基因多态性与肿瘤坏死因子(TNF)-α及中重度阻塞性呼吸睡眠暂停综合征(OSAS)的相关性。方法应用TaqMan-PCR方法检测2010年1月至2012年7月在新疆维吾尔自治区人民医院高血压科住院的408例中重度OSAS患者(OSAS组)和194名健康体检者(对照组)的ZFP36 3个位点(rs251864、rs3746083及rs17879933)。采用放射免疫法检测TNF-α。结果 OSAS组的颈围、腹围、体重指数、收缩压、舒张压、血尿酸、总胆固醇及TNF-α均明显高于对照组(P<0.05)。两组在ZFP36 rs251864和rs17879933位点的基因型和等位基因频率分布差异有统计学意义,TNF-α在上述2个位点的不同基因型差异有统计学意义(P<0.05)。3个位点的不同基因型在呼吸睡眠相关指标中,差异均无统计学意义。Logistic回归分析结果显示,TNF-α不是OSAS的独立危险因素。在控制了年龄、性别等混杂因素后,未发现ZFP36 3个位点基因多态性与OSAS之间存在相关性。结论中重度OSAS患者TNF-α明显增高,同时ZFP36参与对TNF-α的调节,但ZFP36 3个位点的突变不是中重度OSAS发生的独立危险因素。
Objective To investigate the relationship between genetic variations in the ZFP 36 gene and moderate to severe OSAS. Methods Three common single nueleotide polymorphisms (SNPs)of the ZFP 36 gene (rs251864, rs3746083 and rs17879933 ) were detected in patients with moderate to severe OSAS group ( n = 408 ) and control gruop ( n = 194 ) by the TaqMan polymerase chain reaction method. Results The moderate to severe OSA group and the control group showed sig- nificant differences not only in neck circumference, abdomen circumference, BMI, SBP, DBP, UA, TC, and TNF-a, but in the distributions of genotypes and alleles of rs251864 and rs17879933 (P 〈 0. 05 ). The moderate to severe OSAS group and the control group showed significant differences in the distributions of genotypes and alleles of rs251864 and rs17879933 (P 〈 0. 05 ). TNF- a were significantly different among the three genotypes of rs251864 and rs17879933. While there were no sig nificant differences in sleep apnea parameters in the three polymorphisms of the ZFP 36 gene. Logistic regression analysis demonstrated that TNF-a and the three polymorphisms of the ZFP 36 gene were not independently associated with OSAS. Conclusion ZFP36 might involved in the regulation of TNF-a. But ZFP36 gene variants were not the independent risk fac- tors related to moderate to severe OSAS.
