侧支循环对单侧颈内动脉系统血管狭窄病人脑血流储备的影响 被引量:6

摘要 目的利用自体二氧化碳(CO2)吸入法实现结合经颅多普勒超声(TCD)测定单侧颈内动脉系统血管狭窄病人的脑血流反应性(CVR),并初步评价侧支循环代偿情况对脑血流储备的影响。方法选择2008年11月—2009年10月在首都医科大学附属北京天坛医院神经内科住院治疗的单侧颈内动脉或单侧大脑中动脉狭窄或闭塞病人,共29例。所有病例均由数字减景血管造影(DSA)确诊。受试者进行TCD检查,记录CO2激发试验前后双侧大脑中动脉的血流速度,同时监测呼出CO2浓度的变化,根据公式得出脑血流储备。结果对不同等级代偿血流病人的CVR进行统计,发现1级代偿病人CVR与3级代偿病人比较差异有统计学意义(P<0.05)。而1级代偿病人与2级代偿病人的CVR比较差异无统计学意义(P=0.122),2级代偿病人与3级代偿病人CVR比较差异无统计学意义(P=0.117)。结论脑血流储备能力受侧支循环的建立情况影响,侧支血流代偿级别越低,脑血流储备能力受损越明显。
