脑损伤患儿治疗效果及影响因素分析

摘要 目的探讨脑损伤患儿的治疗效果及影响因素。方法回顾性分析我院2010-07—2011-12收治的118例脑损伤患儿的临床资料,根据治疗效果分为疗效良好组(82例)与疗效不佳组(36例),采用χ2检验和Logistic回归分析2组患儿的临床资料,找出影响脑损伤后患儿临床治疗效果的相关因素。结果单因素分析结果显示疗效良好组与疗效不佳组在孕周、出生体质量、脑实质损伤程度、神经行为发育情况、开始治疗年龄比较差异有统计学意义(P<0.05);非条件多因素Logistic回归分析结果显示早产(β=1.573,OR=4.821)、脑实质损伤程度重(β=1.659,OR=5.254)是脑损伤患儿治疗效果不佳的独立危险因素。结论脑损伤后患儿的总体治疗效果较好,但早产及重度的脑损伤患儿的预后常较差,对于这部分患儿,应进行更早、更全面的康复治疗。 Objective To investigate the treatment effect and influencing factors of children with brain injury. Methods The clinical data of one hundred and eighteen children with brain injury in the outpatient service of our hospital from July 2010 to December 2011 were retrospectively analyzed. They were divided into the good curative effect group (82 eases) and poor therapeutic effect group (36 cases) according to the treatment effect. Then their clinical data were analyzed by Chi-square test and binary classification Logistic regression analysis in order to find out the treatment effect and influencing factors of children with brain injury. Results Eighty-two cases had full recovery, thirty-six cases of children had poor recovery. Single factor a- nalysis showed that there were significantly different in the gestational age, the birth weight, the degree of brain injury, neu-robehavioral development situation, starting treatment age between two group. Unconditioned Logistic regression analysis results showed that small gestational age (β=1. 573, OR=4. 821) and brain injury extent (β=1. 659, OR=5. 254) were independent risk factors for the treatment of the brain damage in children. Conclusion The treatment effect on children with brain injury is good, but premature birth and the prognosis of children with severe brain injury is often poor, so this part of the children should be received more early and more comprehensive rehabilitation treatment.
