经颅多普勒在Ⅰ、Ⅱ级高血压患者中脑血管损伤的研究 被引量:2

摘要 目的观察Ⅰ、Ⅱ级高血压病患者中脑血管的血流变化、血管损伤程度。方法应用经颅多普勒超声,对90例Ⅰ、Ⅱ级高血压患者和90例正常人行经颅多普勒超声检查,观察颈内动脉系统的大脑中动脉、大脑前动脉、大脑后动脉的平均血流速度(Vm)、搏动指数(PI)、阻力指数(RI)。结果高血压I级组:颈内动脉系统各分支血管平均血流速度明显加速,脑血管痉挛,PI、RI值正常。高血压II级组颈内动脉系统各分支血管平均血流速度降低,脑供血不足,PI、RI值升高。结论高血压I级组患者处于高血流动力学状态,脑血管痉挛,以大脑中动脉、大脑后动脉发生率最高,大脑前动脉次之。高血压II级组患者,脑血管血流速度减慢,血管阻力加大,同时伴有脑动脉硬化的图形特征。 Objective To quest the bases of early diagnosis and treatment of apoplexy through observing the change of hypertensive' s cerebrovascular hemodynamics and the degree of their vascular injuries by Transcranial Dop- pler. Methods The meanvelocity of blood flow, PI and RI of CL cerebrimedia, a cerbri anterior and a cerebri posterior were measured by Transcranial Doppler in 90 hypertensives and 90 normal persons. Results In hypertension 1 group the mean velocity of blood flow of the branches of eL carotis interna were more accelerative than normal group's and the cere- brovasculars became spasmodic, but the value of PI and R1 was normal. In hypertension 11 group, the mean velocity of blood flow of the branches of a. carotis interna was more decelerative and cerebral ischemic, the value of PI and R1 was higher. Conclusions The patients of hypertension I group were in the hyperhemodynamic condition. Their cerebrovess- ds became spasmodic Among them, the incidence rate of spasm of a cerebrimedia was highest, a. cerebri anterior took the second place In hypertension H group, the patients' cerebrovascular mean velocity of blood flow deeeleiated and cerebra- vascular resistance strengthened, at the same time accompanied sonarographic character of cerebral arteriosclerosis.
