高风险药品管理模式的探索与实践

Exploration and Practice of High-risk Medications Management

目的:探索高风险药品管理模式,规范医院药事管理,保障患者用药安全。方法:阐述了高风险药品的概念及医院施行高风险药品管理的必要性,从机构、目录、储存、使用、监督、教育和宣传等方面介绍了我院建立的高风险药品的管理体系并进行分析和评价。结果与结论:医院加强高风险药品管理非常必要。通过制订高风险药品目录、强化使用环节控制、建立监督改进机制等手段,有计划、分步骤地建立、健全了医院高风险药品管理体系,有效地防范了高风险药品用药差错的发生。

OBJECTIVE： To investigate high-risk medications management model, standardize the hospital pharmacy management and ensure the safety of medications use in patients. METHODS ： The definition of high-risk medications and the necessity of high-risk medications management were described, and high-risk medications management system was introduced, analyzed and evaluated in terms of institutions, list, storage, application, monitoring, education and propaganda. RESULTS ＆ CONCLUSIONS ： It is necessary to strengthen high-risk medications management system. In order to reduce the incidence of medication errors, we should establish high-risk medications list, strengthen the link management of medications use, develop supervision and improvement mechanism and establish and improve high-risk medications management system step by step in a planned way.
