斑点追踪技术对心肌缺血者乳头肌水平室壁运动的研究 被引量:2

摘要 目的探讨超声斑点追踪技术在诊断心肌缺血者节段性室壁运动异常的临床应用价值。方法应用超声斑点追踪技术测量20例心肌缺血患者及20例正常人相对应心肌节段的二维应变值,采集其左室短轴乳头肌水平的二维图像,测量6个节段的收缩期圆周应变值、圆周应变率、局部短轴缩短率,并用SPSS 13.0软件进行统计分析。结果对照组左室短轴乳头肌水平收缩期峰值圆周应变值、圆周应变率、局部短轴缩短率在6个节段间差异无统计学意义;心肌缺血组患者的乳头肌水平6个心肌节段收缩期峰值圆周应变值、圆周应变率、局部短轴缩短率与对照组比较明显减低,差异有统计学意义(P<0.05)。结论超声斑点追踪技术可准确诊断节段性室壁运动异常,为临床评价心肌缺血者左心室局部收缩功能提供一种科学、客观、无创的新方法。 Objective To study ultrasound speckle tracking technology in the diagnosis of patients with myocardial ische- mia segments chamber wall motion abnormalities clinical application value. Methods Ultrasound speckle tracking tech- nology was used to measure 20 cases of patients with myocardial ischemia and 20 cases of normal person corresponding to the heart of sarcomere two-dimensional strain values, the left ventricular short axis papillary muscle level plane 2 d images were collected,6 segmental systolic circumferential strain values the circmnferential strain rate and partial short axial shortening rate were measured and analyzed with spssl3.0. Results Left ventricular short axis of papillary muscle level circumferential strain, circumferential strain rate and local short axial shortening rate were not statistically significant difference between the wall ; cases of patients with papillary muscle level six tleart segmental systolic circumferential strain value of scaromere,circumferential strain rate, local short axial shortening rate were significantly reduced compared with normal control group, there were significantly differences ( P 〈 0.05 ). Conclusion Ultrasound speckle tracking technolo- gy can accurately evaluate segmental ventricular wall motion abnormalities and myocardial ischemia for clinical assessment in patients with left ventricular systolic function, which provided scientific and objective, the new methods of noninvasive.
