摘要
目的:探讨脑脊液中花生四烯酸乙醇胺(arachidonylethanolamide,anandamide,AEA)浓度与精神分裂症患者服药前后的变化及病理机制。方法:采集43例精神分裂症患者服药前后脑脊液样本,高效液相色谱法进行分析测定治疗前后样品中AEA含量,同时用阳性与阴性症状量表(positive and negative symptoms scale,PANSS)评定患者精神症状;并将测定结果和20名正常对照组CSF中AEA含量进行比较。结果:治疗前精神分裂症患者组CSF中AEA含量明显高于对照组(P<0.01)。经药物治疗后患者CSF中AEA浓度显著下降(P<0.01),并与对照组比较无显著性差异(P>0.05)。结论:精神分裂症患者症状活跃期CSF的AEA含量增高,治疗后AEA含量降低,这一变化可能与精神病的病理机制有关,探索大麻系统(endocannabinoid system,ECS)与精神分裂症之间的关系对临床具有一定意义。
OBJECTIVE To study the changes of AEA(arachidonic acid ethanolamine)concentration in cerebrospinal fluids(CSF)of schizophrenics before and after the treatment and the pathological mechanism.METHODS The samples of cerebrospinal fluids of 43 schizophrenics before and after the treatment were collected and analyzed in terms of AEA concentration by adopting high performance liquid chromatography(HPLC);PANSS(positive and negative symptoms scale)was employed to evaluate the patients' mental symptoms.And the measurement results of AEA concentration were compared to those of 20 normal persons(control group)in CSF.RESULTS The results showed that the AEA concentration in CSF of schizophrenics with positive symptoms was little higher than that of patients with negative symptoms,indicating a negligible difference(P0.05). The AEA concentration in CSF of a schizophrenic before the treatment was significantly higher than that of the control group(P0.01),while after the treatment,AEA concentration underwent a remarkable decrease(P0.01)to almost the same level of the control group(P0.05).CONCLUSION This paper draws the conclusion that the AEA concentration increases when symptoms of schizophrenics turn to be active,however,after treatment,AEA level in CSF is negatively correlated with patients' mental symptoms,which perhaps may connected to the pathological mechanism.The study on the relationship between ECS(endocannabinoid system)and schizophrenia has great meaning to clinic.
